Safety of Outpatient Closed-Loop Control: First Randomized Crossover Trials of a Wearable Artificial Pancreas

Abstract: OBJECTIVEWe estimate the effect size of hypoglycemia risk reduction on closed-loop control (CLC) versus open-loop (OL) sensor-augmented insulin pump therapy in supervised outpatient setting.RESEARCH DESIGN AND METHODSTwenty patients with type 1 diabetes initiated the study at the Universities of Virginia, Padova, and Montpellier and Sansum Diabetes Research Institute; 18 completed the entire protocol. Each patient participated in two 40-h outpatient sessions, CLC versus OL, in randomized order. Sensor (Dexcom … Show more

“…49 The superior glucose control achieved by these trials suggests that when CGM information is processed appropriately by advanced algorithms, the clinical outcomes are better, even than those achieved by state-of-the art sensoraugmented insulin pump therapy. 50,51 Thus, CGM data contain valuable information reaching beyond the episodic BG determinations provided by SMBG. At a certain level of CGM accuracy, this information should permit effective ''non-adjunct'' use of CGM as a replacement for SMBG.…”

“…The AP system architecture has been clinically evaluated for three potential configurations, based on the hardware platform where the controller was implemented, specifically: computer/laptop [7,[16][17][18], tablet [8,19,20] and smartphone [21]. In this study, we examine two possible AP hardware configurations; we refer to them as Configuration A and Configuration B.…”

“…Further advances in both sensor performance and algorithms incrementally demonstrated improved protection against hypoglycemia, reduced variability, and decreased mean glucose levels in controlled inpatient settings (11)(12)(13) as well as in outpatient studies (2,4,6). The strategies for different control algorithms are well described in several recent reviews (14,15).…”
