Introduction: Since the introduction of mesh in Inguinal Hernia Repair (IHR) the recurrence rate was lowered. Instead, postoperative pain which may be related to mesh side effects e.g. fibrosis, foreign body reaction, erosion and migration, and infection, has been recognized as the relevant outcome factor. The long-term effects of mesh and composite mesh need to be analyzed.Methods: Pubmed, google scholar search using the search terms: polypropylene mesh, titanized mesh, TiMesh ® ; about polypropylene mesh the following terms: adhesion, erosion, infection, shrinkage, migration, seroma, degradation, pain; biocompatibility, foreign body reaction. Also, a bibliography of relevant articles and product-related information was searched.Results: Mesh properties are determined by tensile strength, pore size, weight, reactivity and biocompatibility, elasticity, constitution, and shrinkage. PolyPropylene Mesh (PP) is associated with actual and long-term complications more than expected. These complications are associated with mesh structure-lightweight meshes showed fewer risks for complication as infection, pain, mesh migration, foreign body response, erosion, adhesion, shrinkage, pain, seroma, and recurrence. TiMesh ® showed an advantage in in-vitro and animal studies which were supported by clinical studies. There is a shorter convalescence, and lesser foreign material sensitivity with comparable results in complications (Chronic pain, infection, recurrence). Conclusion:Polypropylene mesh has been successful in inguinal hernia repair for decades. However, in recent years complications of PP became evident in scientific reports and the public. Complications seem to be related to the material and structure of the mesh but also to the surgeon's expertise and patient's characteristics. It became evident that more complications appear long-term. The true rate of complications could be higher. There are a few clinical reports on titanium-coated mesh (TiMesh) in TAPP and TEP inguinal hernia repair. Experience with TiMesh in laparoscopic procedures long-term and open IHR are missing. Especially about the long-term effects of coating PP with titanium we need to have a better understanding of the interaction of soft tissue and mesh. There are promising results of pure titanium mesh for inguinal hernia repair which need to be confirmed in further controlled clinical studies.

Aim: In this study, it was aimed to compare the results of Total Extraperitoneal (TEP) and Transabdominal Preperıtoneal (TAPP) surgical methods in laparoscopic inguinal hernia repair. Methods: The data of 317 patients who underwent laparoscopic inguinal hernia repair surgery with the diagnosis of inguinal hernia between October 2016 and January 2021 in the General Surgery Clinic of SBU Sultan 2. Abdulhamid Han Training and Research Hospital were evaluated retrospectively. The wound dehiscence, swelling in the incision or scrotum, recurrence, and the postoperative day of returning to work were questioned. The results of TAPP and TEP methods were compared statistically. Results: In our study, the mean age of 317 patients who underwent surgery with TEP and TAPP methods was 50.5, and 27 of them were female and 290 were male. Postoperative recurrence was observed in 21 (6.6%) patients, wound dehiscence in 11 (3.4%) patients, swelling at the incision in 44 (13%) patients, and scrotal swelling in 30 (9.4%) patients. It was determined that the patients who underwent the TEP procedure returned to work after an average of 5.09 (1-30) days, and the patients who underwent the TAPP procedure after an average of 4.04 (1-14) days. When the two groups were compared, there was no significant difference between the groups in terms of return to work day in the postoperative period (p= 0.707), recurrence in the postoperative period (p=0.493), swelling at the incision (p=0.479), scrotal swelling (p=0.356), and wound dehiscence (p=0.245). Conclusions: There are two methods commonly used today in the repair of inguinal hernia by laparoscopic method. TEP method is performed in the preperitoneal area and TAPP method is performed in the intraperitoneal area. In this study, the results of TEP and TAPP methods were compared and it was found that there was no statistically significant difference between the two procedures in terms of postoperative recurrence, time to return to work and postoperative complications.

A study to compare the outcomes of laparoscopic transabdominal preperitoneal patch plasty (TAPP) and open Lichtenstein repair in the management of inguinal hernia was carried out at the Department of Surgery, SKIMS Medical College, Bemina, Srinagar, Kashmir, India, from June 2017 to December 2018. Sixty patients with inguinal hernia were enrolled in the study. It was a type of non-randomized prospective cohort study, wherein 30 patients were treated with TAPP and 30 with open Lichtenstein repair. Patients were studied since admission till discharge and followed for 2 years in the outpatient department. Mean operation time in TAPP was higher than the Open Lichtenstein Repair, but the overall complications were higher in the latter. Patients with inguinal hernias who underwent repair by TAPP approach had significantly better post-operative courses than those who undergo open Lichtenstein repair.
