Sarcoidosis as an Autoimmune Disease

Abstract: Despite the large number of performed studies, the etiology and pathogenesis of sarcoidosis still remain unknown. Most researchers allude to the possible autoimmune or immune-mediated genesis of the disease. This review attempts an integral analysis of currently available information suggesting an autoimmune genesis of sarcoidosis and is divided into four categories: the evaluation of clinical signs described both in patients with sarcoidosis and "classic" autoimmune diseases, the role of triggering factors in… Show more

“…Vimentin-derived peptides are the most plausible candidate for the activation of both T-cells and B-cells in the lung [58]. Autoimmunity presents a as a novel spectrum for sarcoidosis immunopathogenesis and may help elucidate sarcoid etiology [59][60][61].…”

“…One autoantigen was shared by all 3 screening sera of patients with sarcoidosis: Negative elongation factor E (NELF-E; RNA-binding protein RD) ( Figure 2). We compared our results with former protein array screenings, which were performed in former studies [13][14][15][16][17][18]. NELF-E antibodies were not detected in any of these protein screening arrays from patients with GPA (n = .…”

“…Despite numerous attempts, only circumstantial evidence exists to regard sarcoidosis as an autoimmune disease, including its response to immunosuppressive therapy, association with certain Human Leucocyte Antigen (HLA) genotypes and detection of some autoantibodies that are also associated with other autoimmune diseases such as antimitochondrial, antinuclear, anti-double stranded deoxyribonucleic acid (DNA) and citrullinated cyclic peptide antibodies [13].…”

“…Interestingly, B cells might also play a role in the autoimmune phenomena associated with the onset and progression of sarcoidosis. In fact, several types of autoantibodies have been found, both circulating and associated with the lung tissue, in patients with sarcoidosis, such as anti-mitochondrial, anti-nuclear antibodies, and autoantibodies to double-stranded DNA [70]. Case-report evidence of responses to rituximab exists in extrapulmonary sarcoidosis but not for pulmonary disease as yet [71].…”
