Introduction. An intimate relationship between osteoporosis and sarcopenia has been established. At present, there are few epidemiological studies about osteosarcopenia due to the recent use of this term, especially in Latin America. Objective. To study the association between osteoporosis and sarcopenia and determine the prevalence of osteosarcopenia in patients who attended a rheumatology center in Ecuador. Methods. A cross-sectional study was conducted in a population of patients who had a densitometric study. The diagnosis of sarcopenia was determined by the DXA standard gold test, screening, and conventional methods (bioimpedance, anthropometric measurements, SARC-F, muscle function, and gait test). Results. A total of 92 patients were studied. The median age was 66 ± 10, 90% females. Using the criteria of SMI, 65% had sarcopenia of which 9% had only sarcopenia and 56% had osteosarcopenia; 22% had only osteopenia/osteoporosis; and 13% none of these conditions. The prevalence of sarcopenia according to handgrip strength was 60%, gait speed 45%, and SARC-F score 40%. The prevalence of osteosarcopenia according to handgrip strength was 51%, gait speed 34%, and SARC-F score 32%. Osteoporosis was associated with a higher prevalence of sarcopenia using the criteria of SMI since 40% had sarcopenia in the normal DXA group, 64% in the osteopenia group, and 76% in the osteoporosis group (p=0.017). Of the women, 69% had sarcopenia compared to 33% of the men (p=0.034). The BMI was lower in the group with sarcopenia (25.1 ± 4.1 kg/m2) compared to the group without sarcopenia (29.4 ± 4.1 kg/m2, p<0.001). Patients with osteosarcopenia and sarcopenia had lower BMI, handgrip strength, ASM, SMI, and total-body skeletal muscle mass than those with osteopenia/osteoporosis or normal patients. Conclusion. 65% of the studied population had sarcopenia. It is clear that the prevalence of sarcopenia is higher in patients with greater loss of bone mass. Identifying pathways that affect both bone and muscle could facilitate the development of treatments that simultaneously improve osteoporosis and sarcopenia.

Background. Cardiovascular risk (CVR) underlies aging process and longevity. Previous work points to genetic and environmental factors associated with this risk. Objectives. The aim of this research is to look for any CVR gene-gene and gene-multifactorial/lifestyle interactions that may impact health and disease and underlie exceptional longevity. Methods. A case-control study involving 521 both gender individuals, 253 centenarians (100.26±1.98 years), and 268 controls (67.51±3.25 years), low (LCR, n=107) and high (HCR, n=161) CVR. Hypertension, diabetes, obesity (BMI, kg·m-2), and impaired kidney function were defined according to standard criteria. CVR was calculated using Q risk®. DNA was genotyping (ACE-rs4646994, AGT-rs4762, AGR1-rs5182, GRK4-rs2960306, GRK4-rs1024323, NOS3-rs1799983, and SLC12A3-rs13306673) through iPlex-MassARRAY®, read by MALDI-TOF mass spectrometry, and analyzed by EARTDECODE®. Results. Antilongevity factors consisted (OR 95% CI, p<0.05) BMI 1.558 (1.445-1.680), hypertension 2.358 (1.565-3.553), smoking habits 4.528 (2.579-7.949), diabetes 5.553 (2.889-10.675), hypercholesterolemia 1.016 (1.010-1.022), and regular consumption of red meat 22.363 (13.987-35.755). Genetic aspects particularly for HCR individuals ACE II (OR: 3.96 (1.83-8.56), p<0.0001) and NOS3 TT (OR: 3.11 (1.70-5.70), p<0.0001) genotypes were also risk associate. Obesity, smoking, hypercholesterolemia, and frequent consumption of red meat have an additive action to hypertension in the longevity process. There was a synergistic interaction between the endothelial NOS3 genotypes and the severity of arterial hypertension. An epistatic interaction between functional genetic variants of GRK4 and angiotensinogen was also observed. Conclusions. Cardiovascular risk-related genetic and multifactorial or predominantly lifestyle aspects and its interactions might influence the aging process and contribute to exceptional longevity in Portuguese centenarians. Besides lifestyle, the activity of nitrite oxide synthase may be one of the main physiologic regulators of cardiovascular protection in the path of longevity.

Цель исследования: изучение композиционного состава тела у больных ишемической болезнью сердца (ИБС) старше 90 лет (долгожителей) и анализ взаимосвязей между содержанием жировой, тощей ткани и минеральной плотностью костной ткани (МПКТ). Материал и методы. Данная работа-одномоментное («поперечное») исследование, в которое было включено 200 пациентов старше 90 лет (140 женщин и 60 мужчин, средний возраст 92,4 ±2,3 года), госпитализированных с диагнозом ИБС. Композиционный состав тела анализировали посредством двухэнергетической рентгеновской абсорбциометрии. Результаты. Избыточная масса тела или ожирение диагностированы у 139 (69,5%) больных. Скелетно-мышечный индекс оставался в пределах нормальных значений у 145 (72,5%) больных и был ниже нормы у 55 (27,5%). Снижение минеральной плотности костной ткани (Т-критерий) менее-2,5 SD выявлено у 81 (40,5%) больных, нормальные значения МПКТ-у 60 (30,0%) пациентов. Наименьшие значения МПКТ обнаружены в ребрах, наибольшие-в позвоночнике и нижних конечностях. Найдена положительная корреляция между индексом массы тела (ИМТ) и минеральной плотностью костной ткани во всех участках скелета (р <0,0001 для всех областей). Выявлена существенная прямая корреляция между содержанием жировой ткани во всех участках тела и МПКТ, особенно значимая между МПКТ ребер и содержанием жировой ткани в туловище (r=0,85; p <0,0001). Установлена прямая корреляция между содержанием тощей ткани и МПКТ; наиболее достоверная-между МПКТ верхних конечностей и содержанием тощей ткани в верхних конечностях (r=0,69; р <0,0001). Между содержанием жировой и тощей ткани найдена отрицательная корреляция, наиболее значимая между тощей и жировой тканью в нижних конечностях (r=-0,46; р <0,0001). Заключение. Полученные результаты свидетельствуют об особенностях композиционного состава тела у долгожителей. Отмечена достаточно высокая доля больных с избыточной массой тела, но с нормальными показателями МПКТ и содержания тощей ткани. Подтверждены значимые взаимосвязи между костной, жировой и тощей тканью.
