SAT0362 Mycophenolate Mofetil versus Methotrexate in The Management of Large Vessel Giant Cell Arteritis

Abstract: BackgroundThere is an unmet clinical need to identify an effective conventional adjuvant immunosuppressive agent in the treatment of large vessel giant cell arteritis. Traditionally the therapeutic approach has employed prolonged courses of high dose glucocorticosteroids (GC) with poor long-term outcomes. Methotrexate (MTX) is currently recommended as adjuvant GC sparing immunosuppression based on modest evidence1,2. Evidence for the use of Mycophenolate mofetil (MMF) in this setting is lacking.ObjectivesWe in… Show more

“…An initial case series ( n = 3) reported significant reduction in prednisolone dose in all three patients with newly diagnosed GCA treated with mycophenolate . A further retrospective study reported a similar steroid‐sparing effect for mycophenolate ( n = 11) compared to methotrexate ( n = 9) in a group of GCA patients with angiographically proven large‐vessel vasculitis …”

“…However, the evidence supporting their use is largely limited to case series. [29][30][31][32][33][34][35] One small nonrandomized double-blind study of azathioprine in patients with either PMR or GCA showed a significant reduction in mean steroid dose over 52 weeks. 36 An RCT of hydroxychloroquine published in abstract form showed no evidence of efficacy.…”
