Scheimpflug imaging of ultra-late postoperative capsular block syndrome

Abstract: This report highlights the importance of conducting Pentacam Scheimpflug imaging in late CBS. Each case either enhanced our understanding or improved our differential diagnosis of late CBS.

“…Scheimpflug imaging in these cases showed change in the anterior chamber depth and also distention of the capsular bag due to accumulation of fluid in the retrolenticular space. Previous reports also supported our imaging findings that ultra-late presentation usually doesn't show significant change in the refraction or the anterior chamber depth clinically [7].…”

“…Ultra-late onset CBDS is recently described term that presents after 7 years of cataract surgery lacking the classical features of CBDS i.e. shallow anterior chamber, elevated intraocular pressure, and induced myopia from the anterior shift of the intraocular lens [2,7]. Literature shows very few cases that were detected after such a long duration; may be asymptomatic in nature or clinically difficult to diagnose but without any characteristic documentation.…”

“…Additionally, residual lens epithelial cells and posterior capsule opacification were also noted in previous reports [10]. Diagnosis of late-onset postoperative CBS depends on the identifying of milky-white fluids with slit lamp examination and Scheimpflug images [4]. When the liquefied substance is difficult to identify as shown in case 2, misalignment of iris and IOL shown in virtual eye simulation implies unevenly distributed pressure behind IOL and assists the diagnosis (Fig.…”

“…Possible treatments include neodymium-doped yttrium aluminum garnet (Nd:YAG) laser capsulotomy or surgical lysis of adhesions [3]. Scheimpflug imaging has shown advantages in detecting changes in the anterior segment and peri-IOL region including the posterior capsule caused by CBS [4,5]. We present clinical findings of three cases of postoperative CBS diagnosed with the aid of Pentacam Scheimpflug imaging and virtual eye simulation.…”
