Sebaceous carcinoma of the penis: As aggressive as it is rare

Abstract: Malignant neoplasm of the penis is a rare cancer in industrialized countries but is not uncommon in India. Nearly 95% of penile cancer is squamous cell carcinoma. Malignant neoplasms of the sebaceous glands, which are mostly confined to the head and neck region, are even rarer. We present a 74-year-old male with sebaceous cell carcinoma of the penis. He underwent excision of the primary lesion with bilateral inguinal lymph node dissection followed by adjuvant chemotherapy.

“…Sebaceous carcinoma of the penis is a poorly characterized entity with less than 10 verifed cases in the literature. Whereas SC of the eyelids displays a low rate of lymph node involvement and distant metastasis, penile sebaceous carcinoma tends to be metastatic at presentation [3]. It is necessary to consider SC in the diferential diagnosis for penile masses, despite SCC making up 95% of cases [3].…”

“…Treatment options for SC of the penis include local excision, partial penile amputation, and radical penile amputation [3]. For lymph node metastasis and extranodal involvement, a protocol of paclitaxel and carboplatin, used for orbital SC, may be warranted [3,8].…”

“…In few cases, sebaceous gland carcinomas have also been found on the external genitals or on extracutaneous localizations, such as oral mucosa, salivary gland, lung, and breast. [32][33][34][35][36] Clinically, sebaceous gland carcinomas usually present as unspecific reddish or yellow-brown nodules or indurated plaques. They may occur with or without ulceration and reach a size of several centimeters.…”

“…Periocular sebaceous gland carcinomas are typically found on the upper eyelid originating from Meibomian and, more rarely, Zeis glands, or the pilosebaceous unit of periocular hair follicles. In few cases, sebaceous gland carcinomas have also been found on the external genitals or on extracutaneous localizations, such as oral mucosa, salivary gland, lung, and breast 32–36 …”

“…Periokuläre Talgdrüsenkarzinome finden sich typischerweise am oberen Augenlid mit Ursprung aus den Meibom‐ und seltener den Zeis‐Drüsen oder der pilosebozytären Einheit der periokulären Haarfollikel. In wenigen Fällen wurden Talgdrüsenkarzinome auch am äußeren Genitale oder an extrakutanen Lokalisationen wie der Mundschleimhaut, der Speicheldrüse, Lunge und Mamma beschrieben 32–36 …”
