Segmental testicular infarction due to cholesterol embolism: Not the first case, but the first report

Abstract: Reported herein is an unusual case of intrahepatic cholangiocarcinoma with lymphoepithelioma-like appearance in a 64-year-old man who was found to have an intrahepatic mass without cirrhosis. The tumor had two distinct histological patterns with dense lymphoplasmacytic infiltrate. The first was similar to nasopharyngeal undifferentiated carcinoma; the second pattern was a well-differentiated adenocarcinoma. Transition between the two components was observed in the same duct. “…Although thrombi formation itself is similar to usual atherothrombotic emboli, which implies cholesterol embolism characterized by atheromatous debris with cholesterin deposition and macrophage in a variety of organs from atherosclerotic plaque, 15 the difference between the present case and atherothrombotic emboli is considered to be the distribution of the thrombus and the clinical course. Usually, atherosclerotic emboli due to aortic aneurysm occur in kidney, spleen, gastrointestinal tract, and leg skin.…”
