OBJETIVO: Analisar os fatores associados à percepção negativa de saúde em idosos brasileiros. MÉTODOS: Estudo de base populacional com delineamento transversal realizado no ano de 2009, em três cidades do Brasil. A amostra estratificada de forma aleatória simples foi composta de 909 adultos de 60 a 91 anos. Na identificação dos fatores associados com a percepção negativa de saúde (PNS), foi realizada análise bruta e mutivariável por estimativas das Razões de Prevalência (RP), por meio da regressão de Poisson; p < 0.05. RESULTADOS: A prevalência para PNS foi de 49,6%, e após análise multivariada foi observado associação para os homens com o aumento da idade RP = 1,03 (IC = 1,01 - 1,06), risco de desnutrição RP = 1,66 (IC = 1,13 - 2,43) e incapacidade funcional RP = 1,79 (IC = 1,21-1,77). Para as mulheres, o uso superior a dois medicamentos RP = 1,41 (IC = 1,13 - 2,52), hipertensão arterial RP = 1,52 (IC = 1,43 - 1,97) e a incapacidade funcional RP = 1,36 (IC = 1,13 - 1,86). CONCLUSÕES: Quase a metade dos idosos possuem uma PNS, de maneira que ações preventivas de diminuição ao uso de medicamentos e comportamentos que favoreçam melhores condições nutricionais devem estar atreladas às políticas públicas à promoção da autonomia funcional e do bem-estar dos idosos.

Diabetes, Inflammation, and Functional Decline in Older Adults

OBJECTIVE -Age, diabetes, and elevated inflammatory markers independently increase the risk of functional decline. We examined the effect of C-reactive protein (CRP) and interleukin-6 (IL-6) on the incident mobility limitation in older adults with and without diabetes. RESEARCH DESIGN AND METHODS-We analyzed data from a cohort of 2,895 well-functioning adults aged 70 -79 years, followed for development of persistent functional limitation over 3.5 years. Participants were assessed for the presence of diabetes according to fasting glucose and/or hypoglycemic medication use and were divided into three equal groups (tertiles) according to level of CRP or IL-6. Persistent functional limitation was defined as difficulty climbing 10 steps or walking one-quarter mile on two consecutive semiannual assessments.RESULTS -At baseline, 702 participants (24%) had diabetes. CRP values were (median Ϯ SD) 2.8 Ϯ 4.4 versus 3.7 Ϯ 5.4 for those with normal glucose and diabetes, respectively (P Ͻ 0.001). The unadjusted incidence of functional limitation associated with increased levels of CRP and IL-6 was greater among participants with diabetes. After adjusting for clinical and demographic covariates, persistent functional limitation for the highest tertile was greater compared with that for the lowest tertile of CRP or IL-6 for those with and without diabetes. CRP hazard ratios (HRs) were 1.7 (95% CI 1.2-2.3) versus 1.4 (1.1-1.6), respectively. IL-6 HRs were 1.8 (1.3-2.5) versus 1.6 (1.4 -2.0), respectively.CONCLUSIONS -In initially high-functioning older adults, those with diabetes and higher inflammatory burden had an increased risk of functional decline. Interventions at early stages to reduce inflammation may preserve function in these individuals. The Bambuí health and ageing study (BHAS): methodological approach and preliminary results of a population-based cohort study of the elderly in Brazil Estudo de Bambuí sobre saúde e envelhecimento: metodologia e resultados preliminares de coorte de estudo de idosos no Brasil Abstract ObjectiveA cohort study has been designed to identify predictors of adverse health events in the elderly. The methodology of the study and preliminary descriptive results are presented. MethodsThe study population comprises all residents of Bambuí (Minas Gerais, Brazil), aged 60 or more years (n=1.742). From these, 92.2% were interviewed and 85.9% underwent clinical examination, consisting of haematological and biochemical tests, serology for Trypanosoma cruzi, anthropometric and blood pressure measures and electrocardiogram. Aliquots of serum, plasma and DNA were stored for future investigations. The baseline interview included sociodemographic characteristics, self-referred health condition and history of selected diseases, medication use, health service use, source of medical care, physical activities, smoking, drinking and eating habits, reproductive history, physical functioning, life events, social support and mental health. Individuals are being followed up annually. ResultsThe following characteristics predominated among participants: women (60,0%), married (48.9%) or widowed (35.4%), people living in households with up to 2 residents (73.8%), heads of family (76.7%), people with monthly income between 1.00 and 2.99 Brazilian minimum wages (62.0%) and people with up to 4 years of schooling (89.1%). The median age was 68 years. Among the cohort members, only 1.7% were lost in the first follow-up. ConclusionsIn general, the characteristics of the study population were very similar to those from other epidemiological studies of the elderly based on large Brazilian cities. The small number of losses to follow-up indicates that the choice of Bambuí was adequate, assuring the feasibility of a long term cohort study. Resumo Objetivo
