Sensibility of the rectum to distension and the anorectal distension reflex in ulcerative colitis.

Abstract: SUMMARYThe maximum volume of air tolerated within a rectal balloon was less in colitic patients than in normal subjects (p < 0 001 Urgency of defaecation, sometimes leading to incontinence, is one of the most disabling symptoms experienced by a patient with colitis. The studies of rectal distension reported here show that many patients with colitis are unable to tolerate normal volumes within the rectum. The reflex relaxation of the internal sphincter on distension of the rectum, is apparently unaffected by th… Show more

“…Rectal sensation was tested by inflating a latex balloon with air at 1 ml/s and determining the threshold volumes for first constant sensation, defecatory desire volume (DDV), and maximum tolerable volume (MTV). 46 Patients were considered to have RH if MTV was ,100 ml in females or ,80 ml in males (determined in 56 healthy control subjects). Pudendal nerve terminal motor latencies (PNTML) were recorded with the St Mark's pudendal stimulating electrode (Dantec Electronics Ltd, Bristol, UK).…”

“…Several disturbances of gut function have been demonstrated in IBD, such as changes in sensory perception (Farthing & Lennard-Jones, 1978;Rao et al, 1987) and changes in gut motility (Snape et al, 1980;Loening-Baucke et al, 1989;Koch et al, 1990;Annese et al, 1997;Chrysos et al, 2001). These ®ndings re¯ect both in¯ammation-induced changes in the responsiveness of the smooth muscle and nerves.…”

“…In a number of patient groups, diarrhea is associated with increased rectal tone, [18][19][20][21] and we hypothesized that the same was true for patients with the carcinoid syndrome. The effect of 5-HT on rectal compliance remains obscure and needs further study.…”

“…Our hypotheses were based on the clinical presentation of patients with carcinoid diarrhea and on previous data from patients with diarrhea caused by ulcerative colitis, irritable bowel syndrome, or sequelae of radiation therapy. [18][19][20]31 Excessive production of 5-HT escaping hepatic first-pass metabolism is considered the main reason for the carcinoid syndrome. 5 As seen in most patients included in the present study, very high levels of 5-HT can be reached in the systemic circulation of patients with NET.…”
