Sensitivity and specificity of the Distress Thermometer and a two‐item depression screen (Patient Health Questionnaire‐2) with a ‘help’ question for psychological distress and psychiatric morbidity in patients with advanced cancer

Abstract: Ultra-brief screening tools offer an efficient means of identifying patients with advanced cancer with severe distress or psychiatric morbidity but are less effective at identifying non-distressed individuals. Used in conjunction with a 'help' question, these tools can help clinicians identify patients who are both distressed and likely to accept professional support.

“…The DT has been validated in patients with many different types of cancer, has good sensitivity and specificity, and shows good concordance with the Hospital Anxiety and Depression Scale as a measure of emotional difficulty [3•]. A two-item depression screen (Patient Health Questionnaire [PHQ]-2) has been shown to be effective in identifying patients with severe distress or psychiatric morbidity in cancer settings [5]. Screening measures, regardless of length, are best used to identify patients who may require further assessment and intervention; they should not be used alone as diagnostic tools [6].…”

“…On arrival to the Ambulatory Infusion Center, a social worker or a trained volunteer offered the iPads along with basic guidance on how to use the device for guided meditation. Patients recorded their stress levels pre-and post-chemotherapy via the "Distress Thermometer" [8] making marks between 0 (no distress) and 10 (severe distress). Data was only recorded on patient's first use of the device during chemotherapy.…”
