Sensitivity of ICD-10 diagnosis of psychotic disorders in the Israeli National Hospitalization Registry compared with RDC diagnoses based on SADS-L

Abstract: In 87% to 89% of cases with psychotic disorders or with schizophrenia, Registry diagnoses agreed with RDC diagnoses, a rate of agreement comparable with those of other, similar registries. Because a large number of analyses derived from this and similar national registries will be published in the coming years, this constitutes relevant information.

“…This is determined by a committee, including a psychiatrist and recognized by National Insurance Institute regulations. The last registry hospital discharge diagnosis of schizophrenia, of whom all Israeli rehab service users are part of, has acceptable sensitivity and specificity assessed against research diagnosis (57); has acceptable stability over time (58); and captured over 90% of cases with schizophrenia disorders based on research criteria in a community sample (59). Inclusion criteria were: men and women over the age of 18, who had been psychiatrically hospitalized in the past, had experienced psychotic symptoms according to their clinicians and corroborated by the Positive and Negative Syndrome Scale (PANSS) [(60) –see measures] clinical interview (administered by the first author) and were eligible to sign an informed consent.…”

“…Run by the Ministry of Health since 1950 (Lichtenberg, Kaplan, Grinshpoon, Feldman, & Nahon, 1999), it contains a record of all admissions to psychiatric hospitals or psychiatric wards within general hospitals, as well as admissions to day-facilities for psychiatric treatment; it includes dates of admission and discharge and a single discharge diagnosis for each episode, assigned by a board-certified psychiatrist. These diagnoses are coded with the International Classification of Diseases (ICD); codes from earlier years have been update to 10th Revision and those for psychotic disorders have been validated recently (Weiser et al, 2005). …”
