Serous macular detachment associated with optic nerve pits

Abstract: Between September 1972 and March 1989, we examined 25 eyes of 24 patients exhibiting optic nerve-head pits associated with serous macular detachment. We followed 21 patients for a period of 1-15 years (mean, 62 months). Various treatment modalities were applied, depending on the time of initial examination. Five eyes underwent no treatment and three eyes underwent two distinct treatment modalities. Photocoagulation treatment was performed in ten eyes. Argon laser photocoagulation was combined with intravitreal… Show more

“…Serous macular detachment or schisis develops in B25-75% of patients with visual deterioration. 3 Although spontaneous reattachment has been reported, 4,5 the prognosis of ODPM is poor if left to its natural course. 6 Several therapeutic approaches have been attempted, but responses have been variable.…”

“…However, the precise origin of the sub-retinal fluid remains elusive. Several possible sources have been suggested, including cerebrospinal fluid, liquefied vitreous, 3,14 or leaky vessels at the base of the pit. 2 Postel et al 15 also observed some cases with a subtle tear in the diaphanous tissue overlying the optic pit, suggesting a rhegmatogenous component to the detachment.…”

“…Serous macular retinal detachment is associated with 25% to 75% of optic nerve head pits (2)(3)(4) . Using the cross-sectional retinal imaging provided by optical coherence tomography (OCT), several authors have confirmed the concept of a bilaminar structure with schisis-like retinal changes overlying a more central neurosensory retinal detachment (5)(6)(7) .…”
