We studied bc(a-2 microglobulin (β-2Μ) levels in 44 HTV infected subjects belonging to 3 clinical stages as well as in 25 healthy controls. The method used was a competition enzyme immunoassay. In this study, levels of β2-Μ were measured in two groups of HIV infected individuals, the asymptomatic and those with progressive and advanced disease, in order to affirm its role as a surrogate prognostic marker. It was found that mean p-2M levels were 1.28 mg/L in the controls (normal range 0-2.4 mg/L), 11.41 mg/L in the HIV infected subjects, 2.69 mg/L in the asymptomatic HIV infected, 12.14 mg/L in those with persistent generalized lymphadenopathy (PGL) and 39.29 mg/L in the patients with acquired immunodeficiency syndrome. It was concluded that β-2Μ levels were significantly higher in the HIV infected as compared to the controls. Further, the levels were much higher in the IIIV infected with progressive disease/PGL and highest in those who had developed AIDS. β-2Μ is an important surrogate serological marker useful in prognostication of disease process in the HIV infected. Advantages of measuring B-2M levels over p24 antigen detection and CD4 counts were highlighted. MJAFI1997; 53: 251-252

ResumenCon el fin de determinar el comportamiento de las diferente poblaciones linfocitarias, a través de los diferentes estadios clínicos de la infección por elvirus de inmunodeficiencia humana (VIH) establecidos por el CDC, se realizaron tipificaciones de linfocitos T, B, T CD4 y T CD8 en 155 individuos VIH positivos del ISS en Cali, utilizando la técnica de inmunocitoquimica de los complejos fosfatasa alcalina-antifosfatasa alcalina (APAAP) en extendidos de sangre periférica.En los pacientes del estadio II, los linfocitos T, B y CD4 se encontraron dentro de los valores normales para la población general, mientras que los CD8 estaban aumentados. En el estadio III, el comportamiento de las poblaciones de linfocitos fue similar salvo un ligero aumento de los linfocitos B. Estos estadios sólo se diferenciaron clínicamente por la presencia de adenopatias.En el estadio IVse observó una marcada depleción detodas las poblaciones linfocitarias estudiadas.En virtud al aumento temprano de CD8 en los estadios II y III, la relación CD41CD8 presenta alteraciones y es menor de 1 ,O aunque el recuento de CD4 en estos estadios sea normal. Sin embargo, cuando ocurre ladepleción general en el estadio IV, su utilidad es limitada y el recuento absoluto de CD4 es el indicador más útil en la progresión de la enfermedad.En virtud a que el recuento de linfocitos CD4 y CD8 constituye un pilar y en él se orientan la mayoría de las conductas profilácticas y de tratamiento que se adoptan en los VIH positivos, se hace necesario conducir los esfuerzos para hacer más asequible esta metodología a los pacientes, así como plantear la implementación de otras pruebas evaluadoras de la inmunidad, útiles como indicadores de la progresión de la enfermedad. Objective: To evaluate the clinical utility of urinary b-2-microglobulin (B2M) at birth, an alternative to proinflammatory cytokines, as an indicative marker of fetal inflammatory response and subsequent higher risk of bronchopulmonary dysplasia (BPD) in premature infants.Study Design: The relationship between urinary B2M at birth and the occurrence of BPD was examined in 96 premature infants with a description of perinatal backgrounds. Constructing a receiver-operating characteristic curve to determine the cutoff value of urinary B2M at birth for the development of BPD, a multivariate logistic regression analysis was performed to evaluate whether elevated urinary B2M at birth can be used as a predictor of BPD.Results: BPD was diagnosed in 34% (33/96) of the infants. Neonates with BPD had a significantly higher occurrence rate of chorioamnionitis and greater levels of median urinary B2M at birth than did those without BPD. The selected cutoff value of urinary B2M at birth correlated with the development of BPD, even after adjusting for gestational age and other confounding factors.Conclusions: Elevated urinary B2M levels at birth can be used as an alternative marker of fetal inflammatory response and subsequent higher risk of BPD in premature infants.
