Service evaluation of faecal immunochemical testing and anaemia for risk stratification in the 2‐week‐wait pathway for colorectal cancer

Abstract: Background New national guidance on urgent referral for investigation of colorectal cancer included faecal occult blood testing in 2015. A service evaluation of faecal immunochemical testing (FIT) and anaemia as risk stratification tools in symptomatic patients suspected of having CRC was undertaken. Methods Postal FIT was incorporated into the colorectal cancer 2‐week wait (2WW) pathway for all patients without rectal bleeding in 2016. Patients were investigated in the… Show more

“…In this dataset the proportion of FIT results of < 4.0 μg Hb/g faeces is higher than during our pilot , confirming that direct GP access to FIT does enable ‘opportunistic screening’. This is further evident in the wide age range of patients who returned a FIT kit, with patients as young as 18 being tested.…”

“…We have previously described our straight to test (STT) pathway and our FIT pilot . A number of locally agreed modifications were required to integrate FIT into clinical pathways (Appendix in the online Supporting Information): Primary care colleagues had direct access to FIT.…”

“…Thus, the existing DG30 system and test‐kit allocation could be scaled up to include patients with high risk symptoms. A referral system based entirely on FIT will miss cancers; but with a negative predictive value ranging between 99·1 and 99·8 per cent this may be deemed acceptable in the current environment. The London pathway involves referring only FIT‐positive patients at a threshold over 10 μg Hb/g faeces for investigation; patients with more than 150 μg Hb/g faeces should be prioritized for colonoscopy.…”
