Shift of tumor features in patients with clinically localized prostate cancer undergoing radical prostatectomy since the beginning of the PSA era

Abstract: During the 12-year period, PC was increasingly detected on the basis of a pathological PSA level only and shifted significantly to more organ-confined stages. With a time delay, these findings are consistent with trends observed in large centers in the USA.

“…However, in the last decade, a stage migration of PC has been observed. PC has been detected increasingly due to a pathological PSA level only and has shifted significantly to more OC stages (31,32). Consequently, tumors with ECE and with seminal vesicle invasion have decreased distinctly.…”

“…As a result of improvements in diagnostic procedures and increased screening in most countries, prostate cancer is currently usually diagnosed at the earlier stages (i.e. more organconfined disease) than previously (211)(212)(213) . Epidemiological studies that have examined the association between obesity and risk of prostate cancer have provided conflicting results.…”

“…Currently, more men are presenting with well-differentiated tumors, low PSA levels, and nonpalpable disease. This dramatic shift in presentation, which may be due to PSA use, has caused a major stage migration for prostate cancer, with nearly 80% of newly diagnosed cases revealing only localized disease [11,12]. In patients with localized prostate cancer, radical prostatectomy is the most effective treatment tool and offers the best chance of a cure [13].…”
