Sialadenoma papilliferum with potentially malignant features

Abstract: Sialadenoma papilliferum (SP) is a benign non-invasive lesion with slow and limited growth. This report describes an unusual SP that enlarged rapidly and presented as an extensive intraoral tumour highly suspicious for verrucous carcinoma. Radiographic resorption of the underlying bone was evident in the mandible. This type of destructive growth does not conform to any previously reported SP. Histological features were also atypical: surface verrucous hyperplasia and deeper florid cystic-duct adenoma. Overall,… Show more

“…Although there have been a few reports of recurrence and malignant transformation, in which SP has reported that it can transformed into epithelial-muscle epithelial carcinoma, squamous cell carcinoma, and mucoepidermoid carcinoma, but malignant transformation is rare and not entirely convincing [6,16,32,47]. As the results of the statistics show that the prognosis of this lesion is very good, we believe that SP does not have malignant potential [14,44,48].…”

“…The clinical findings for UKCD cases are summarized in Table 1 and are combined with the literature review cases in Table 2. Case reports alleging malignant transformation [8,[32][33][34][35] were excluded from our review because, in our opinion, there was insufficient evidence in the publications to support origin from a preexisting SP.…”

“…Cases of malignant transformation of SP have appeared in the literature; [8,[32][33][34][35] however, in our opinion, there is insufficient evidence in the publications to unequivocally determine whether any of the malignancies truly originated within a pre-existing SP. Although the case reported by Ide et al [8] may represent an SP with potentially malignant features, we could not entirely exclude a papillary cystadenocarcinoma with a surface papillomatosis based on the available photomicrographs. Surface papillomatosis (papillary epithelial hyperplasia) previously has been described overlying other malignant salivary gland neoplasms, most often polymorphous adenocarcinoma [41].…”

“…The present SP size matches the one reported in the literature, as it does not exceed 2 cm (16). The majority of the cases reviewed (41 out of 50, 82%) match this criteria; however, five cases included SP lesions bigger than 2 cm (1,13,14,31,37).…”
