Silk suture granuloma with false-positive findings on PET/CT accompanied by peritoneal metastasis after colon cancer surgery

Abstract: HighlightsSuture granuloma is relatively rare. After tumor resection, positive PET/CT findings could indicate suture granuloma.Postoperative PET/CT surveillance might give false-positive findings.PET/CT cannot detect small peritoneal metastatic lesions.A surgical examination should be performed when metastases are suspected.

“…Suture granulomas have shown increased uptake of 18 F-fluorodexyglucose, probably because of the presence of granulation tissue associated with chronic inflammation, during positron emission tomography (PET)/CT examinations in previous studies, thus leading to false-positive results [ [10] , [11] , [12] , [13] ]. In many previous reports, patients have received unnecessary extended resection or harmful chemotherapy for suspected malignancies.…”

“…In women who have undergone surgery for ovarian cancer, foreign body granulomas mainly formed around the suture materials or clips. Silk sutures cause more allergic reactions and infections than monofilament sutures, and foreign body granulomas occur more frequently in Asia where silk sutures are used more frequently [ 22 ]. Foreign body granuloma is not well detected on CT or MRI because it is radiopaque.…”

“…15 Its worthwhile to note that suture granulomas can present as enhancing masses on MRI or CT and can be associated with increased uptake at F-18 fluoro deoxy glucose (FDG)-Positron Emission Tomography (PET)/CT imaging. 16,17…”
