Simulated first-phase insulin release using Humulin or insulin analog HIM2 is associated with prolonged improvement in postprandial glycemia

Abstract: We examined the extent to which priming the liver with a pulse of Humulin or the insulin analog hexyl-insulin monoconjugate 2 (HIM2) reduces postprandial hyperglycemia. Somatostatin (0.5 g ⅐ kg Ϫ1 ⅐ min Ϫ1 ) was given with basal intraportal insulin and glucagon for 4.5 h into three groups of 42-h-fasted conscious dogs. From 0 -5 min, group 1 (BI, n ϭ 6) received saline, group 2 (HI, n ϭ 6) received a Humulin pulse (10 mU ⅐ kg Ϫ1 ⅐ min Ϫ1 ), and group 3 (HIM2, n ϭ 6) received a HIM2 pulse (10 mU ⅐ kg Ϫ1 ⅐ min Ϫ… Show more

“…These patients maintained a second-phase insulin response both before and after BPD. In a more experimental approach, simulating the first-phase insulin response in diabetic patients using exogenous insulin delivered to the liver improved postprandial glucose homeostasis with effects that last four hours post-infusion 5 .…”

“…The net glycogen synthetic rate (or net hepatic carbon retention) was calculated as NHGU À net hepatic lactate output, as previously explained in detail [41]. For all glucose balance calculations, glucose concentrations were converted from plasma to blood values by using previously determined [5,42] correction factors (the mean of the ratio of the blood value to the plasma concentration), which have been published previously [43]. In this report, blood glucose concentrations were thus used for the calculation of NHGB because the use of whole blood glucose ensures accurate hepatic balance measurements regardless of the characteristics of glucose entry into the erythrocyte.…”

“…The relative bioavailabilities of the 0.25 mg/kg (41.5 nmol/kg) oral dose of liquid tregopil (Part 2), of the 6 mg (0.27 mg/kg, 45.0 nmol/kg, mean) oral dose of tregopil in tablet form (Part 3), and of the inhaled and subcutaneous insulin doses (Part 4) were estimated by comparison to data from a previous study . In the prior experiment, DiCostanzo et al administered a bolus of human insulin into the portal vein of six dogs over 5 minutes at a rate of 10 mU/kg/min (300 pmol/kg).…”

“…25 by comparison to data from a previous study. 26 In the prior experiment, DiCostanzo et al administered a bolus of human insulin into the portal vein of six dogs over 5 minutes at a rate of 10 mU/kg/min (300 pmol/kg). This study was chosen because the plasma arterial insulin concentration vs time curves were similar to the tregopil protocols.…”
