Simultaneous Double Eyelid Blepharoplasty and Ptosis Correction with a Single-Knot, Continuous, Nonincisional Technique: A Five-Year Review

Abstract: There are no previously published articles documenting simultaneous double eyelid blepharoplasty and ptosis correction, using a single-knot continuous nonincisional technique. The authors suggest our simple and effective method is a good option for double eyelid blepharoplasty in cases with mild-to-moderate blepharoptosis.' LEVEL OF EVIDENCE 4: Therapeutic.

“…More Asians with single eyelids receive blepharoplasty to build charming double eyelids to satisfy increased aesthetic demands for appearance. In contrast to Caucasian upper eyelids which have a high fold and depressed shape, Asian eyelids are bloated and droopy due to anatomic structure, such as hypertrophy septum fat and insufficient terminal LPS aponeurotic fibers interdigitated with pretarsal skin and OOM [ 18 , 19 ]. Therefore, excising the prolapsed septum fat is key to forming a stable and deep fold in traditional Asian blepharoplasty [ 20 ].…”

“…It is also problematic in terms of the longevity and accuracy of repair because the muscles are shortened by suspension to themselves, rather than to an externally stable structure. Moreover, most nonincisional methods lack measured placement of a tagging suture on the Müller’s muscle, and as such there is no way of precisely quantifying the extent of muscle tucking 7…”

“…Moreover, most nonincisional methods lack measured placement of a tagging suture on the Müller's muscle, and as such there is no way of precisely quantifying the extent of muscle tucking. 7 Here, we report a relatively simple suturing route that, by applying a tagging suture before manipulating the Müller's muscle, prevents penetration of the needle out through the opposite skin layer. Moreover, by placing the tagging suture, the surgeon is able to adjust the amount of Müller's muscle tucking according to the degree of ptosis.…”

“…Asians have a more prominent preseptal fat distribution, variable position and depth of the superior palpebral fold, and insufficient or absent terminal interdigitations of levator aponeurotic fibers onto the pretarsal skin and orbicularis compared to Caucasians (1,2). Therefore, some Asians have a single upper eyelid, upper eyelid hypertrophy, or downward growing eyelashes, which makes their eyes appear small and droopy (2,3). Thus, double eyelid blepharoplasty is one of the most commonly performed cosmetic surgical procedures and has become increasingly popular in Asia, especially in young women, for modifying the eyes with a pair of attractive double eyelid (4,5).…”
