Single embryo transfer: Why and how to identify the embryo with the best developmental potential

Abstract: Multiple pregnancies with higher risk of preterm birth and the associated higher morbidity have been a major obstacle from the early days of in vitro fertilization. A good strategy to avoid multiple pregnancies is elective single embryo transfer and cryopreservation of spare embryos. Important factors in adopting this strategy are good counselling of the patients and the selection of embryos with high implantation potential. Technical advances in embryo selection have been described during recent years, time l… Show more

“…Embryo quality is primarily assessed through an embryo grading system where embryos are graded according to their morphological appearance and are given a score that relates to their development at the time of assessment [10]. The goal is to provide a more precise estimate of implantation potential [11].…”

“…Multiple pregnancies pose a potential health risk for the mother, including pregnancy-induced hypertension and postpartum haemorrhage [9]. Hence, several countries (such as Finland, Sweden, Belgium, Australia and New Zealand) have employed elective single embryo transfer to reduce the possibility of multiple gestation and eventual complications [10,11]. However, many countries are yet to adopt the strict regulations in ART practice, presumably due to the expense involved coupled with the patient's hope to achieve pregnancy in each cycle with better chances.…”

“…Although implantation is a complex process that depends on a coordinated cross-talk between the endometrial factors and the embryo itself, a proper embryo quality is one of the factors that play an important role in embryo implantation [48]. Moreover, the bioactivity of platelets is one of the several factors (genomic, transcriptomic, proteomic, metabolomic, The Cause of infertility male factor 28% 32% cytokines, growth factors, hormones, and the embryo itself) that are involved in the mechanism of endometrial receptivity [49].…”
