Background: Minimal access surgery has become the mainstreams of breast surgery, and it is applied for benign lesion, partial mastectomy and total mastectomy. Dual axillary-areolar incision was the most frequent surgical approach in the early stage, but single axillary incision hybrid technique was proposed in recent years. Endoscopic nipple sparing mastectomy was usually applicable to patients with small to medium sized breast. Here, we reported two cases of breast cancer patients, who presented with large and ptosis breast, underwent single port 3-dimension (3D) endoscopic nipple sparing mastectomy (E-NSM) followed by pedicled transverse rectus myocutaneous (TRAM) flap breast reconstruction. Case presentation: The first patient was a 42-year-old woman had palpable right breast mass and enlarged axillary lymph nodes for 4 months. Core needle biopsy(CNB) revealed invasive ductal carcinoma of breast and lymph node metastasis. Breast Magnetic Resonance Imaging(MRI) revealed multi-centric masses in right upper breast, about 6.1 cm in size, without nipple and skin invasion, and right axillary lymphadenopathy. Neoadjuvant chemotherapy was suggested, but the patient refused and preferred surgery first. She underwent single-port 3D E-NSM with immediate pedicled TRAM flap reconstruction. The post-operative recovery was uneventful except partial nipple ischemia/necrosis, which recovered after 2 to 3 months of wound care. The second case was a 39-year-old woman had one palpable mass over right breast for one month. CNB showed ductal carcinoma in situ with focal infiltrating ductal carcinoma. MRI revealed multi-focal mass, 8.7 cm in size, without skin or nipple invasion and no lymph node metastasis. NSM was suggested, but she decided to undergo single-port 3D E-NSM with immediate pedicled TRAM flap reconstruction due to previous history of Cesarean section and obese abdomen. The post-operative recovery was uneventful. Conclusion: Single-port 3D E-NSM followed by pedicled TRAM flap reconstruction is a feasible, safe procedure with satisfactory cosmetic outcome. Time-consuming and technical demanding are two limiting factors of this procedure.

Backgroud: Longer follow-up was necessary to testify the exact value of mastoscopic axillary lymph node dissection (MALND).Methods：From January 1, 2003 to December 31, 2005,1027 patients with operable breast cancer were randomly assigned to two groups: MALND and CALND. 996 eligible patients were enrolled. The end points are disease free survival and overall survival.Results：The final cohort of 996 patients was followed for an average of 184 months. The distribution of all events was fairly similar between two groups of patients. The incidence of local in-breast events did not differ in a significant manner between two cohorts. Similarly, the rate of distant metastases was not significantly different with 30.0% in MLND and 32.6% in CALND. And no significant difference was observed in other primary tumor between two groups (p=0.46). Patients who remain alive with no event comprise a total of 37.2% in MALND and 35.4% in CALND. Other primary cancers and deaths from other causes were distributed equally between two groups. The 15-year disease-free survival rates were41.1 percent for the MALND group and 39.6 percent for the CALND group (p=0.79). MALND was found to be not inferior for overall survival (P =0.54). The 15-year overall survival rates were 49.5 percentafter MALND and 51.2 percentafter CALND (p=0.86). Probability of overall survival was not significantly different between two groups.Conclusions：MALND does not increase unfavorable events, and also does not affect the long-term survival of patients. Therefore, MALND should be one of the preferred approaches for breast cancer surgery.

Most patients are asymptomatic while symptoms ranging from mild discomfort to severe pain can present in patients with gynaecomastia. In addition to these, this condition may affect the psychological well-being of patients leading to a need for further treatment. Medical treatment of primary gynaecomastia in the form of anti-oestrogen therapy has not been proven to be effective and there is no consensus regarding the drug of choice or optimal duration of treatment. Surgical treatment is usually the standard treatment in primary gynaecomastia. There have been various techniques described in the literature with the aim of restoring a pleasant chest shape with limited scar on incision. Most of the techniques however involve the use of a peri-areolar or a Wise pattern incision, which can be obvious, especially in patients with a tendency to scar badly. The authors describe a novel approach, whereby a single-port endoscopic subcutaneous mastectomy using the three-dimensional endoscopic system with incision placed along the anterior axillary line was performed for a patient with gynaecomastia and thereby conferring excellent aesthetic outcomes.
