Sinus Surgery in Bone Marrow Transplantation Patients

Abstract: Bone marrow transplantation (BMT) is used in the treatment of a wide range of malignancies. It involves immunosuppression and increased risk of infections such as sinusitis. However, the risk of infection is much higher in allogeneic transplants than those who are replaced with their own bone marrow (autologous). Immunodeficient patients may develop fungal sinusitis with a high mortality rate. An aggressive approach to evaluation and surgical intervention for fungal sinusitis in BMT patients at the Indiana Uni… Show more

“…The likelihood of the patient developing rhinosinusitis in the first two years after BMT is estimated to be up to 36.9%, according to Savage et al, going against findings of 49% of frequence in this paper. According to numerous authors [8][9][10][11][12] , otorhinolaryngological physical exam alone is not enough to properly assess these transplanted patients. Nasal endoscopy and paranasal sinus CT scan before BMT are ideal and fundamental tests used for the precise and early diagnosis.…”

“…Invasive sinusitis caused by Aspergillus is also a concern in HSCT patients, particularly when associated with neutropenia [59,60]. This condition is less common in solid organ transplant recipients.…”

“…This condition is less common in solid organ transplant recipients. It carries a high mortality, particularly with invasion of bone and spread to the orbit and brain [1,59,60]. In transplant recipients who have invasive Aspergillus sinusitis, a chest CT should be done to exclude pulmonary involvement.…”

“…28 The study concluded that, in patients with a worsening clinical trend despite targeted medical therapy, close endonasal monitoring must be continued; antral biopsy with trocar and culture examination must only be performed after endoscopic examination. Subsequent surgical intervention is only indicated in carefully selected patients.…”
