Small intestinal CMV disease detected by capsule endoscopy after allogeneic hematopoietic SCT

Abstract: Figure 1 Total colonoscopy revealed a single ulceration (a) with several erosions in the ascending colon. Capsule endoscopy also revealed a single ulceration (b) with several erosions in the jejunum very similar to the one found in the ascending colon.

“…Therefore, endoscopy allows direct vision of the mucosa of the alimentary tract, identification of ulcerations and lumen narrowing, and obtaining biopsy specimens for histopathological study . Especially for suspicious small intestine disease, capsule endoscopy may aid in suggesting the diagnosis, but false‐negative results can occur because it is impossible to obtain biopsies . Currently, several reports describe the value of modern alternative imaging methods in the diagnosis of terminal ileitis, such as PET‐CT with 18‐FDG and magnetic resonance enterography .…”

“…After primary infection and persistence, it may be reactivated. Infection often remains asymptomatic but severe disease may occur in immunosuppression, e. g. in malignant diseases, pharmacological treatment or Acquired Immunodeficiency Syndrome (AIDS) [336,337]. It may affect all portions of the gastrointestinal tract, sometimes causing mid-gastrointestinal bleeding, obstruction or perforation [338,339].…”

“…However, biopsies cannot be performed during WCE, and despite the apparently high NPV in these studies, they are based on a very low number of patients and EGD with biopsies remains indispensable. Furthermore, WCE probably lacks specificity given that erosion and ulceration were reported on WCE performed in patients with CMV diseases after allo-SCT [52, 57]. Overall, WCE realization can be useful in patients with GI acute GVHD symptoms and normal EGD.…”
