Soft-Tissue Coverage of the Elbow

Abstract: The elbow requires a durable and pliable soft-tissue envelope. Injuries, infections, or tumors that result in exposed tendon, bone, or implants require expedient soft-tissue coverage. Familiarity with various options for coverage can help tailor treatment to the defect size and minimize donor-site morbidity. In this article, emphasis is placed on muscle and fasciocutaneous flaps that have been shown to have reliable vascularity and predictable outcomes.

“…Soft tissue coverage of joints is always a challenge for surgeons; managing elbow and knee defects is difficult; thus, pedicled muscular flaps are frequently the first choice of surgical teams (Andre et al, 2014; Corten et al, 2013; Helgeson et al, 2007; Milcheski et al, 2014; Rehim et al, 2014; Stevanovic and Sharpe, 2013) and remain available even following failure of a propeller flap. In our review of available cases, no significant difference was highlighted in this review ( p = 1; OR = 1.32; 95% CI: 0.274–8.628) between this location and others in terms of complication rate.…”

“…Sin embargo, requieren un lecho receptor bien vascularizado y ferulizaciones prolongadas para evitar la contractura secundaria 27 . Además, el injerto no provee una cobertura duradera apropiada que resista los movimientos de flexión y extensión repetidos, lo que puede llevar a ulceraciones o erosiones frecuentes 28 . Los colgajos locales son útiles para el tratamiento de secuelas cicatrizales leves o bridas con tejido adyacente sano disponible.…”

“…Similar local flaps available for the elbow and arm when local skin is healthy and viable including advancement flaps and Z-plasties [ 21 ]. Rhomboid flaps are useful around the elbow when the defect is small and the defect can be excised in a triangular shape [ 22 ].…”
