Solitaire AB Stent-Assisted Coiling of Wide-Necked Intracranial Aneurysms

Abstract: Stent-assisted coiling of wide-necked intracranial aneurysms was found to be safe and effective with the Solitaire AB stent at 6-month follow-up. Angiographic results improve with time due to progressive thrombosis of the aneurysm.

“…6,20,21 Furthermore, stent-induced neointimal overgrowth leads to healing of the aneurysm neck, promotes progressive thrombosis, and reduces the risk of recanalization. 22 Gory et al 23 reported that 30.9% of aneurysms with partial immediate occlusion evolved into a complete occlusion during a 6-month follow-up, and the recanalization rate was only 14.5%. The progressive occlusion rate in another study was 69.7% after a mean follow-up of 8.2 months.…”

“…4,10 Similar results concerning safety have also been reported by using stent-assisted coiling of wide-neck aneurysms. [11][12][13][14] In a multicenter study reporting the use of the Neuroform stent (Stryker Neurovascular, Kalamazoo, Michigan), 12 permanent morbidity and mortality rates were 1%, respectively, at 12-to 18-month follow-up. In another article, treatment-related permanent morbidity was 1.6% and mortality was 0% by using the Solitaire AB stent (Covidien, Irvine, California).…”

“…In the recent large multicenter series of 85 patients, morbidity and mortality at 1 month were 1.3% and 0.0%, respectively. 18 Wide-neck intracranial aneurysms can be treated with stent-assisted coiling 4,5 ; however, in case of bifurcation aneurysms such as anterior communicating artery aneurysms, the stent placement procedure requires a Y-or X-configuration, which has a high incidence of adverse events and outcomes. [11][12][13] Compared with intracranial stent placement, the major advantage of using a WEB device is the absence of antiplatelet therapy.…”

“…Balloon-assisted and stent-assisted coiling may be therapeutic options. [3][4][5] However, low rates of initial angiographic occlusion and high rates of recurrence are reported in such situations. [5][6][7] In addition, bifurcation aneurysms often require an X-or Y-configuration double-stent treatment technique that carries a higher rate of procedural complications [8][9][10][11][12][13] in addition to the complications linked to the mandatory use of dual antiplatelet therapy.…”
