The effective management of spent fuel pool (SFP) safety has been raised as one of the emerging issues to further enhance nuclear installation safety after the Fukushima accident on March 11, 2011. SFP safety-related issues have been mainly focused on (a) controlling the configuration of the fuel assemblies in the pool with no loss of pool coolants, and (b) ensuring adequate pool storage space nto prevent fuel criticality owing to chain reactions of the fission products and the ability for neutron absorption to keep the fuel cool. In support of regulatory functions, the Centre for Nuclear Safety and Security (CNSS) seeks to perform confirmatory analysis for all potential accident scenarios that may occur in the Koeberg nuclear power plant SFP. Probabilistic safety assessment (PSA) was done using the Systems Analysis Program for Hands-On Integrated Reliability Evaluations (SAPHIRE) computer code. We present preliminary PSA results of initiating events that lead to boiling and cause fuel uncovering, resulting in possible fuel damage in the Koeberg nuclear power plant SFP.

The crew of a nuclear-powered submarine under development in Brasil plays an important role in its safety. For this reason, crew members must be trained in procedures against undesired events. However, information that would help to accurately decrease human error probability (HEP) on board nuclear-powered submarines is not available in the literature. Therefore, a methodology is required to obtain this information. This study introduces importance measures to evaluate the contribution of performance shaping factors (PSFs) and their levels to HEP. Importance measures Fussell-Vesely, risk reduction worth, risk achievement worth, and Birnbaum importance, commonly used in probabilistic safety assessment, are introduced to identify PSFs and PSF levels that contribute the most to HEP. The proposed approach is focused on identifying HEP's main contributors, oriented toward decision-making.The methodology uses the PSFs established in the Standardized Plant Analysis Risk-Human Reliability Analysis method and a Bayesian network to quantify HEP. Moreover, the results of an application case demonstrate the method's effectiveness in integrating expert opinions into representative values of HEP and in identifying critical PSFs and PSF levels that should be the object of the decision-making process to improve human performance.

In the context of the applications of higher automation and complex system configuration in nuclear power plants (NPPs), ensuring nuclear safety by maintaining the reliability of the operators in main control room is become more crucial than ever before. As an essential dimension of physiological indicator, human mental workload is widely used in evaluating human performance and reliability. This paper presents an approach to evaluate the mental workload of operators using multiple resources in Visual, Auditory, Cognitive, and Psychomotor (VACP) model, and takes time pressure (TP) as a critical index. We have performed the approach by a real case study and analyzed the relationship between the operator mental workload and TP, task type and the error probability during the Loss of Coolant Accident (LOCA) under full power operation. There are two operating phases in the workload has been analyzed. Moreover, the relationship between human‐error probability (HEP) and TP had been studied via volatility gain analysis for distinguishing which step of a task is of most important. The results shows that the fault remind (“informing LOCA”) is the first important fluctuation in the phase ignoring TP and HEP. On the other hand, the information validity (“ensuring control rods has been inserted to the bottom”) in the case considering TP is the most important. Because the high task load is positively correlated with human error, the research provides an important basis for preventing human errors in the phase of NPP designs, which will be especially useful in safety‐critical tasks.
