Splanchnic Cortisol Production Occurs in Humans

Abstract: Glucocorticoids are potent regulators of protein, fat, and carbohydrate metabolism. To determine if cortisol production occurs within the splanchnic bed in humans, 11 nondiabetic subjects were studied using the hepatic/ leg catheterization method along with an infusion of [9,11,12,12-2 H 4 ] cortisol (D4-cortisol) as proposed by Andrews et al. In the fasting state, there was net release (P < 0.05) of cortisol from the splanchnic bed (6.1 ؎ 2.6 g/min) and net uptake (P < 0.05) by the leg (1.7 ؎ 0.7 g/min). This… Show more

“…15,16 This extra-adrenal source contributes to peripheral cortisol levels and potentially affects the selectivity index. Furthermore, a previous study showed that admixture of blood from the accessory hepatic veins into AVs lowers the selectivity index of cortisol.…”

“…At ϳ0900 h, H]glucose and a hormone cocktail containing somatostatin and replacement amounts of growth hormone and glucagon were started (time 0 min) and continued until the end of the study as part of a separate experiment (14). Insulin was infused at a rate of 0.78 mU ⅐ kg lean body wt Ϫ1 ⅐ min Ϫ1 from 0 to 180 min, then at 1.56 mU ⅐ kg lean body wt Ϫ1 ⅐ min Ϫ1 from 181 to 300 min, and then at 3.1 mU ⅐ kg lean body wt Ϫ1 ⅐ min Ϫ1 from 301 to 420 min.…”

“…Previous attempts to measure adipose 11HSD1 in vivo using arteriovenous sampling across abdominal subcutaneous adipose tissue have been inconclusive, perhaps because of variability in measurement (26). Recently, we (27) and others (28) have validated a new approach using deuterated cortisol tracer infusion to measure the turnover of 11HSD activities in vivo. Here, we use this technique to quantify systemic regeneration of cortisol by 11HSD1.…”

“…Here, we use this technique to quantify systemic regeneration of cortisol by 11HSD1. Recent studies using arteriovenous sampling have suggested that the splanchnic circulation (liver and intra-abdominal adipose tissue) accounts for most of the systemic regeneration of cortisol (28). To assess cortisol regeneration specifically in subcutaneous adipose tissue, we have developed a new technique using in vivo microdialysis.…”
