Splenosis: Ultrasound and CT findings in a case complicated by an intraperitoneal implant traumatic hematoma

Abstract: A case of intraperitoneal splenosis, found incidentally at ultrasound examination in a 35-year-old man, is reported. The diagnosis, suspected on the basis of computed tomographic (CT) and radionuclide studies, was established by surgery. Three years later, after direct trauma, ultrasound and CT studies revealed a surgically proven splenosis implant hematoma. The contribution of radiologic examinations in the diagnosis of splenosis is emphasized.

“…There is a very limited role for ultrasound in splenosis as imaging characteristics are nonspecific, with lesions often appearing as a mass with uniform echogenicity, either hyper-or hypoechoic depending on the surrounding tissue [7]. The typical splenosis implant measures about 1 cm and can demonstrate mild hypertrophy over time.…”

“…Splenosis rarely causes symptoms and is often incidentally found at surgery for unrelated problems or post-splenectomy adhesions, at autopsy, or during diagnostic imaging tests (AGHA 1984;DELAMARRE et al 1988). Symptoms associated with splenosis vary and include: gastrointestinal hemorrhage (when b d tissue implantation occurs in the stomach or small bowel); repeated hemoptysis in thoracic splenosis; abdominal pain due to intraperitoneal nodule torsion with infarction, bowel obstruction or traumatic rupture or hematoma of a splenule; or flank pain from ureteral compression and hydronephrosis.…”
