Spontaneous Adrenal Hemorrhage in Pregnancy: A Case Series

Abstract: Background. Abdominal pain during pregnancy has a broad differential diagnosis which includes spontaneous adrenal hemorrhage (SAH). There is scant literature available on optimal mode of delivery in stable patients. Cases. Patient 1 was a 35-year-old nullipara who presented at 36 weeks of gestation with left flank pain. Patient 2 was a 27-year-old multipara at 38 weeks who presented with left upper quadrant pain. Diagnosis of SAH was made by CT scan and both were managed with pain control, serial hemoglobin as… Show more

“…Although pregnancy is a known risk factor for adrenal haemorrhage (10,11), the risk is unclear (28). It may occur without additional factors and is reported in complicated pregnancies such as hyperemesis gravidarum and difficult labour (11,28,29). One patient in our case series had history of post-partum haemorrhage 15 weeks before presentation.…”

“…We reviewed 5 case reports [8] , [9] , [10] , [11] , [12] and a case series (with 2 cases) [13] for SAH in pregnancy. Most of these presented in third trimester.…”

“…Arterial embolization of adrenals may be needed as a bridging therapy for subsequent surgery in severe hemorrhage cases. In our reviewed 7 case reports [8] , [9] , [10] , [11] , [12] , [13] , all the cases were managed conservatively; none required surgery for adrenal hemorrhage. The cases with adrenal insufficiency were managed with steroids.…”

“…Surgical, conservative, and interventional treatments are the main management of spontaneous adrenal hemorrhage in pregnancy, which depends upon the week of gestation, amount of bleeding, imaging findings, adrenal function, and so on. [69] It is difficult to form a systematic evaluation system for this condition because lack of high evidence studies. Keizer et al [6] put forward if the bleeding is less severe and the patient is hemodynamically stable, conservative management can be appropriate during pregnancy, since their patients achieved good therapeutic effects by just surveillance.…”

[8,9] While Anagnostopoulos and Sharma [1] reported another case of retroperitoneal hematoma secondary to right adrenal hemorrhage and managed by laparotomy drainage and packing of the retroperitoneal hematoma. The laparoscopic approach performed in this case is based on the unconfirmed diagnosis, risk of rehemorrhagia, and relieving of anxiety.…”
