Sporadic and familial cases of aquagenic keratoderma

Abstract: Systemic acitretin may be an effective agent in the treatment of aquagenic keratoderma, and topical application of 10% salicylic acid and 10% urea-containing lotion did not prevent recurrence.

“…Aquagenic wrinkling of the palms (AWP), first described by Elliot in 1974 in patients with cystic fibrosis (CF), is a rare condition that manifests with transient translucent, oedematous papules and plaques and hyperwrinkling on palms and fingers, rarely on soles, after water contact (Table ). AWP has been reported as either recessive or dominant trait. It occurs in up to 84% of patients with CF, which is due to biallelic mutations in the CF transmembrane conductance regulator ( CFTR ) gene (Table ) and characterized by a classical diagnostic triad of chronic lung disease, exocrine pancreatic insufficiency and abnormally high salt concentration in the sweat.…”

“…AWP usually occurs on the palms, but uncommon presentations have been described. For example, unilateral palmar eruption and facial involvement have been separately reported after brief water exposure . It has also been noted that AWP can occur with the sparing of palms, involving only the lateral margin and the volar aspects of the 10 fingers, or even with complete sparing of the palmar aspect of the hand and involvement of the dorsal side only …”

“…For example, to reduce AWP in patients with hyperhidrosis, topical aluminum hydroxide (most commonly used) or botulinum toxin injections (in refractory cases) or even tap water iontophoresis (as a last resort) have been tried with good results . Nonetheless, antihistamines, 5% salicylic acid ointment, topical corticosteroids, retinoids, acitretin, and 20% urea‐containing lotion were used with various results …”
