Squamous Cell Carcinoma Arising in Chronic Osteomyelitis in Foot and Ankle

Abstract: In treating recalcitrant ulcers that have not responded to conventional modes of therapy, malignancy should be ruled out and a biopsy done. The treatment of choice for squamous cell carcinoma is amputation. Routine regional lymphadenectomy at the time of amputation seems unnecessary, but regional lymphadenopathy persisting for 3 months after amputation warrants surgical intervention.

“…Nevertheless, investigators point out that carcinomas can also occur in scars overlying quiescent osteomyelitis (e.g., in burn scars) [135]. The aforementioned neoplasms are largely localized in the tibia [135] but also in the foot and ankle [137], in the patella [138] and in a toe [139].…”

“…19 The incidence of Marjolin’s ulcer is higher in developing countries with limited medical resources, whereas it occurs in 1.6% to 23% of all patients with chronic osteomyelitis. 30-32 Marjolin’s ulcers mainly involve aggressive squamous cell carcinomas (SCC), with a latent period of 27 to 30 years from onset of osteomyelitis to malignant transformation. The long duration of chronic osteomyelitis is the single most important predictive factor.…”
