Standardization of presurgical language fMRI in Greek population: Mapping of six critical regions

Abstract: Background Mapping the language system has been crucial in presurgical evaluation especially when the area to be resected is near relevant eloquent cortex. Functional magnetic resonance imaging (fMRI) proved to be a noninvasive alternative of Wada test that can account not only for language lateralization but also for localization when appropriate tasks and MRI sequences are being used. The tasks utilized during the fMRI acquisition are playing a crucial role as to which areas will be activated. Recent studies… Show more

“…All responded favorably to extensive frontal lobe resections, despite widespread epileptic activity recorded by surface and intracranial EEG pre-and postoperatively; an epileptic encephalopathy phenotype, in the first years of life, should not discourage such a resection. EasyCap GmbH, Herrsching-Breitburnn, Germany, 31 Ag/AgCl electrode EEG cap) for both adult and 2/3 pediatric cases, Language-task FMRI 13 (the two adult cases) DTI, and neuropsychological evaluation were available for the two adults and one child. Following phase I investigation, adult cases underwent a phase II, invasive EEG study, with combinations of subdural grid and strips, for purposes of documentation and delineation of epileptogenic zone, as well as Electrocortical Stimulation (ECS), for eloquent cortex mapping.…”

“…Prolonged surface video‐EEG monitoring (Micromed Plus Evolution Systems) with collodion‐applied electrodes according to the 10–20 SI, plus additional electrodes according to the 10–10, in two parasagittal (F1/C1/P1 left, F2/C2/P2 right) and two inferior temporal chains (F9/T9/P9 left, F10/T10/P10 right). Controlled reduction of antiepileptic medicines was undertaken until typical seizures were recorded; structural MRI (1.5 T Siemens Avanto‐Fit, epilepsy protocol, T1 MPRAGE 1 mm iso/blade dark‐fluid/TSE TIR, T2 dark‐fluid 1 mm iso/blade dark‐fluid coronal and axial/blade/TSE dark‐fluid), EEG‐FMRI (Brain Products GmbH, Munich, EasyCap GmbH, Herrsching‐Breitburnn, Germany, 31 Ag/AgCl electrode EEG cap) for both adult and 2/3 pediatric cases, Language‐task FMRI 13 (the two adult cases) DTI, and neuropsychological evaluation were available for the two adults and one child.…”

“…All data were acquired from July 2018 to January 2022 in St. Luke's Hospital, Thessaloniki, Greece. Twenty healthy volunteers were recruited for the evaluation of the fMRI task protocol ( 18 ) and included in this study as the control group. All healthy subjects provided written consent for their participation.…”
