Stapedectomy in Patients With Small Air‐Bone Gaps

Abstract: Controversy exists concerning stapedectomy for patients with small air-bone gaps. The purpose of this study was to examine the results for patients who had a stapedectomy to correct a small (10 dB or less) air-bone gap. One hundred fifty-four patients with suspected otosclerosis were explored and a stapedectomy was performed in 136 (88.3%) of these cases. The mean pure-tone average (PTA) improved 16.7 dB and overdosed the preoperative bone conduction PTA by 8.1 dB. The majority of the stapedectomy patients (89… Show more

“…It is clear that the laser allows for a more gentle surgery than other mechanical techniques. Furthermore, our data are consistent with the observations reported by Lippy et al [1997] in a retrospective study of 136 patients who underwent a stapedectomy to correct an sABG (10 dB or less). The majority of patients (89.7%) had a puretone average closure greater than or equal to 0 dB.…”

“…The current literature on the impact of the preoperative ABG gap at the time of surgery is limited [Lippy et al, 1997;Salmon et al, 2015], and to the best of our knowledge, this is the first study reporting long-term hearing results after stapedotomy in patients with otosclerosis and small ABGs. Performing or delaying stapes surgery remains a controversial topic involving 3 issues: incidence of complications, effectiveness in improving hearing, and stability of long-term results in hearing improvement.…”

“…ABGs within the range of 10-25 dB are considered small ABGs [Wiet et al, 1986;Lippy et al, 1997;Salvinelli et al, 2003;Salmon et al, 2015]. It has been suggested that surgery performed in patients with otosclerosis and preoperative small ABGs could result in better postoperative hearing outcomes [Hüttenbrink, 2003;Marx et al, 2011] and improved quality of life [Subramaniam et al, 2006].…”

“…El objetivo de la cirugía de la otosclerosis (estapedectomía/estapedotomía) es conseguir el cierre del umbral diferencial auditivo (UDA) mediante la eliminación de los focos de osificación anómalos en la platina y reinstaurar la correcta transmisión del sonido, con una alta tasa de éxito 10,11 .…”
