Stevens Johnson Syndrome Initiated by an Adverse Reaction to Trimethoprim-Sulfamethoxazole

Abstract: The authors present a case of a 54-year-old male who presented to the ED with Stevens Johnson syndrome (SJS) beginning on his upper lips, then spreading to his glans penis, airway, and buttocks. After using trimethoprim-sulfamethoxazole (TMP-SMX) to treat a pilonidal cyst diagnosed seven days prior to presentation, the patient began to have desquamating lesions on his upper and lower lips. “…These data confirm previous studies that TMP-SMZ hepatotoxicity is more likely to occur in malnourished patients with untreated advanced HIV infection. Previous studies have also demonstrated an association of TMP-SMZ with Stevens-Johnson syndrome, toxic epidermal necrolysis, and drug eruption with eosinophilia and systemic symptoms (22,23). However, only 2 of our 105 cases had Stevens-Johnson syndrome with neither having HIV infection and none had drug eruption with eosinophilia and systemic symptoms.…”
