Studies on the Pathogenesis of Subacute Thyroiditis

Abstract: HLA typing for A, B and C loci (class I antigens) was performed on 89 patients with SAT (males: 7, females: 82) aged from 23 to 66 years, and DR and MT loci (class II antigens) were investigated in 55 patients. In 12 patients, viral antibody titers were measured as soon as possible after onset and two weeks later. The viral antibodies evaluated were those of Influenza A and B, Coxsackie A9, B1, B2, B3, B4, B5 and B6, Echo 3, 7, 11 and 12, Parainfluenza 1, 2, 3 and 4, and Adeno 8 virus. The following results we… Show more

“…All of the previously published reports have demonstrated correlations either by comparison with much smaller control groups than ours, or only in individual cases with no data against which to compare [11][12][13][14][15][16][17][18][19]. The frequency of HLA-B*35 in SAT patients varied from 67 to 72.5% [12,20,21]. We compared our 60 SAT patients with 1023 healthy subjects, demonstrating an even stronger association, with the frequency exceeding 81% in SAT subjects as compared to being only 10.2% in the control group.…”

“…The presence of HLA-B35 has been observed in about 70% of SAT patients [12,20,21], but the other possible associations suggested so far have not been definitively confirmed. However, it seems doubtful that in HLA-B35-negative patients, SAT would occur in the absence of any genetic susceptibility.…”

“…A definite association between the occurrence of this disease and the presence of HLA-BW 3 5 has been demonstrated in European Caucasoids (Nyulassy et al, 1977), Chinese (Yeo et al, 1981), and Japanese patients (Kobayashi et al, 1985). In Japanese patients, the frequency of CW3 and DRW8 was also significantly higher than in the control but no increase in the frequency of A26 in subacute thyroiditis compared with normal subjects was reported (Kobayashi et al, 1985). In the present study, B35, C3 and A26 were positive in all 3 patients.…”

“…On the basis of these observations, we suggested that a retrovirus may be involved as an etiologic agent in Graves' disease. It is noteworthy that a strong clinical and pathologic association between Sjögren's syndrome and chronic autoimmune thyroiditis has been made in Japanese patients (Kobayashi et al, 1985).…”
