Patients affected with type 1 diabetes and a non-negligible number of patients with type 2 diabetes are insulin dependent. Both the injection technique and the choice of the most suitable needle are fundamental for allowing them to have a good injection experience. The needles may differ in several parameters, from the length and diameter, up to the forces required to perform the injection and to some geometrical parameters of the needle tip (e.g., number of facets or bevels). The aim of the research is to investigate whether an increased number of bevels could decrease forces and energy involved in the insertion–extraction cycle, thus potentially allowing patients to experience lower pain. Two needle variants, namely, 31 G × 5 mm and 32 G × 4 mm, are considered, and experimental tests are carried out to compare 3-bevels with 5-bevels needles for both the variants. The analysis of the forces and energy for both variants show that the needles with 5 bevels require a statistically significant lower drag or sliding force (p-value = 0.040 for the 31 G × 5 mm needle and p-value < 0.001 for 32 G × 4 mm), extraction force (p-value < 0.001 for both variants), and energy (p-value < 0.001 for both variants) during the insertion–extraction cycle. As a result, 3-bevels needles do not have the same functionality of 5-bevels needles, show lower capacity of drag and extraction, and can potentially be related to more painful injection experience for patients.

A finales del año 2019 se desarrolla la pandemia de COVID-19, una enfermedad infecciosa causada por un virus previamente no identificado, que produjo consecuencias catastróficas a nivel mundial. Los reportes epidemiológicos mostraron que después de adquirir la infección, la mayoría de las personas experimentan enfermedad leve sin complicaciones, sin embargo, algunos de los pacientes, requirieron hospitalización. Durante los años 2020 a 2022, fueron evaluados en nuestra institución cerca de 1800 pacientes. Dentro del arsenal terapéutico, fueron utilizados equipos de ventilación mecánica invasiva y no invasiva y medicamentos que incluyeron: antivirales, oxígeno, sedantes, antibióticos, anticoagulantes, protectores gástricos, analgésicos, hipolipemiantes, antiarrítmicos, diuréticos, hipotensores, antipsicóticos, analgésicos, hemoderivados, colchicina, esteroides, fibrinolÍticos y anticuerpos contra interleukina 6: tocilizumab, suplementos vitamínicos entre otros. De igual manera fueron utilizados, catéteres venosos y arteriales, tubos endotraqueales, sondas nasogástricas y de aspiración, tubos de drenaje, sondas vesicales y colocación en posición prona en aquellos pacientes con hipoxemia severa. La terapéutica no estuvo exenta de complicaciones. Nuestro objetivo es hacer del conocimiento a la comunidad médica de algunos pormenores de estos eventos ocurridos tanto en la sala de hospitalización como en la unidad de cuidados intensivos COVID, no atribuidos a efectos directos perjudiciales del virus, sino, a las consecuencias de los tratamientos aplicados.

Objectives This research aimed to determine postpartum females’ self-reported adherence to and experience with short-term thromboprophylaxis using enoxaparin injection, after counselling by pharmacists. It also sought to assess their knowledge of thromboprophylaxis, injection techniques, and confidence in self-injecting. Methods This prospective cohort study was conducted at a public tertiary hospital in Malaysia from March to June 2023. Self-injection-naïve postpartum females who were initiated on thromboprophylaxis and counselled by a pharmacist were conveniently sampled. Knowledge regarding thromboprophylaxis, injection readiness, and technique were assessed one day after the counselling session. A telephonic interview was conducted at the end of the 10-day therapy to determine adherence and adverse effects experienced. Key findings A total of 259 subjects were successfully followed up, with 87.6% (n = 227) adherent to the therapy. Nonadherence was predominantly due to forgetfulness; four had their treatment withheld due to bleeding. One-third of subjects experienced localised pain and bruising. Subjects answered a median of 5/7 knowledge questions and recalled a median of 8/10 injection steps correctly, with those who read the information leaflet provided after counselling scoring significantly higher (P = .02). The majority declared moderate confidence in their ability to self-inject. Subjects who intended to self-inject (P < .01) and were more confident (P = .02) demonstrated better injection technique. Conclusions Postpartum females counselled by pharmacists largely adhered to short-term enoxaparin for thromboprophylaxis. The impact of the counselling session may be enhanced by addressing their psychological readiness to self-inject, awareness of adverse effects identification, mitigation, and management, as well as setting reminders to prevent forgetfulness to inject.
