Sublingual drug delivery system was a well-established platform for delivering the drug that need to exhibit quick action without any first-pass metabolic effect, but various pitfall in the regular sublingual drug delivery systems such as a tablet, capsule etc., that can be overcome by novel sublingual drug delivery technology such as particulate sublingual spray. Sublingual spray for drug administration has gained attention in the market since it proved its propensity to by-pass the first-pass metabolism and to initiate a rapid onset of action due to its atomized micro particulate nature, which is significantly higher than the other sublingual formulations. The approval of sublingual spray by the regulatory agencies may commence further research in this field by subsuming various essential drugs for indications such as cancer pain, cardiovascular issue etc. This article comprises a detailed study on the drawback of conventional sublingual drug delivery system and the approach by which these drawbacks can be overruled by spray technology in the sublingual area and the, in-depth mechanism of drug delivery through sublingual with the help of atomization followed by the entire formulation strategy along with evaluation and industrial perspective of the sublingual spray dosage form as a future tool of patient-friendly drug delivery.

Костная ткань состоит из неорганических компонентов и органического матрикса. Последний состоит из колла-геновых белков (90%), преимущественно -из коллагена I типа, а также неколлагеновых белков [1]. Неорганические вещества кости представлены главным образом фосфата-ми и кальцием, однако, в значительном количестве также присутствуют бикарбонаты, натрий, калий, цитрат, магний, карбонат, фосфор, цинк, барий и стронций [2]. Кальций и фосфор формируют кристаллы гидроксиапатитов, име-ющих формулу Ca 10 (PO 4 ) 6 (OH) 2 . Коллагеновые и неколлаге-новые белки формируют каркас для отложения гидроксиа-патитов. Такое взаимодействие и обеспечивает прочность и упругость костной ткани [3].Кальций -один из основных макроэлементов орга-низма человека. 99% кальция находится в костной ткани, остальной содержится во внеклеточной жидкости и в дру-гих тканях. Кальций поступает в организм при потреблении продуктов питания, наибольшее его количество содержит-ся в молочных продуктах. Выведение кальция происходит через кишечник, почки и кожу, в связи с чем необходимо поддерживать его поступление извне. Всасывание каль-ция из кишечника -процесс, на который влияет множе-ство факторов: состав пищи, возраст, уровень витамина D в крови, генотип рецептора витамина D. Пищевые волокна и оксалаты (соли и эфиры щавелевой кислоты) снижают всасывание кальция, а белки (ароматические аминокис-лоты) и глюкоза -увеличивают. Поваренная соль и кофеин увеличивают экскрецию кальция с мочой, как и высокое потребление белка вследствие увеличенной кишечной аб-сорбции [4].Для нормального всасывания кальция в кишечнике необходим достаточный уровень витамина D в крови. Ги-повитаминоз D широко распространён в мире, он ассоци-ирован не только с отрицательным кальциевым балансом и снижением минерализации костной ткани, но и с мышеч-ной слабостью, приводящей к высокому риску падений, что может приводить к возникновению переломов, осо-бенно у лиц, страдающих остеопорозом [5].В данной статье описана роль кальция и витамина D в поддержании минеральной плотности костной ткани (МПК), а также в профилактике и лечении остеопоро-за, приведены данные исследований по потреблению This article considers the role of calcium and vitamin D in body functions, with focus on skeletal system. We reviewed the results of studies on calcium and vitamin D supplementation worldwide and in Russia. According to these studies, there is insufficient dietary intake of these nutrients, irrespective of geographical, ethnic features and physiological conditions. We also reviewed the data on safety of calcium supplementations for urinary and cardiovascular systems, regimens and doses for vitamin D deficiency treatment in various age groups. The article also contains data on calcium, vitamin D and combined medications, available in Russia, for prevention and treatment of osteoporosis, with highlight on combined ones for better compliance and more convenient dosage frequency.

Background of the study: Third molar surgical removal is a common procedure in oral and maxillofacial surgery, causing moderate to severe pain, swelling, and trismus. Postoperative pain control improves patient recovery and oral function, with commonly used analgesics including Ibuprofen, Aceclofenac, Ketorolac, Tramadol, Paracetamol, Nalbuphine, Nimusulide, and Buprenorphine. Aim and Objective: The study compares oral and sub-lingual Piroxicam for managing post-surgical pain after extraction of impacted third molars, focusing on analgesia onset, pain intensity, swelling, and trismus degree. Materials and method: A randomized control trial was conducted on 30 patients who underwent surgical removal of mandibular impacted third molars. The patients were divided into two groups: Group I (Study) and Group II (Control). All patients underwent the procedure by a single surgeon. Post-operative medications were prescribed according to the study design, with Piroxicam 20mg administered sub-lingually (Group I) and 20mg orally (Group II). The parameters were assessed and measured for both groups, and the results and statistical analysis were compared. Result: The results revealed that the inter-group difference for the post-operative symptoms evaluated using a t-test for Equality of means by comparing the pre-op, POD 2, POD 7 values was statistically significant. Mean onset time taken and pain perception by Group I (Sub-lingual) in the first dose was 32.3 minutes and for Group II was 51.21 minutes which was established to be significantly (P<0.05) low for the sub-lingual group by statistical analysis. Conclusion: The study found that the sublingual route of administering Piroxicam had a faster onset of action and better analgesia postoperatively compared to the oral route. This is crucial for patients and surgeons to improve compliance and comfort post-operatively after impaction surgery, as potent analgesics are essential for effective pain management.
