Subtype Progression and Pathophysiologic Deterioration in Early Schizophrenia

Abstract: It is important to elaborate what we know about the symptomatic, syndromal, and functional course of schizophrenia in order to test models for this illness. The sample of schizophrenic patients from the Chestnut Lodge followup study was subtyped using classical (modified DSM-III-R) criteria and deficit/nondeficit (Schedule for the Deficit Syndrome) criteria. During the first 5 years of manifest illness, the subtype phenomenologies were moderately stable. Instability consisted of a drift toward disorganization … Show more

“…As a caveat, it has long been noted that levels of symptoms of psychotic illness change to varying degrees with time, 29,30 for example, positive symptoms tend to diminish with increasing duration of illness, while negative symptoms are either stable or increase. Genetic studies of psychiatric illness examining either clinical features or clinically defined groups of subjects are therefore both susceptible to measurement error resulting from the temporal instability of clinical features and/or medication effects.…”

“…41 The deterioration occurs predominantly in the early stages of the illness mostly in the first 5 years. 42 Some investigators have even suggested that the prepsychotic phase is the initial phase of deterioration. 43 Kraepelin's initial descriptions of the deteriorating course of schizophrenia are particularly relevant since they are based on the observations prior to the discovery of antipsychotic drugs.…”

“…This figure shows that negative symptoms, cognitive dysfunction, and mood impairment all play a greater role than positive symptoms in social and vocational dysfunction and the overall outcome of schizophrenia. The severity of negative symptoms and cognitive impairments have been shown to be strong determinants of the functional outcome of patients, especially the ability of patients to go back to work or school (Fenton and McGlashan 1991;McGlashan and Fenton 1993). Thus, the greatest opportunities for improving functional outcomes in schizophrenia may result from the development of drugs that lead to improvements in negative symptoms and cognitive deficits.…”
