Successful Aspirin Prophylaxis in a Child with Chronic Paroxysmal Hemicrania

Abstract: It is generally recognized that indomethacin prophylaxis is the treatment of choice in adults with chronic paroxysmal hemicrania (CPH). Our case report is the first to demonstrate complete control of CPH in a 9 year-old child using small dose aspirin prophylaxis. This case also represents direct observation of the youngest patient with CPH having the earliest reported onset.

“…Multiple case reports of children with PH add support to the use of indomethacin in treating this disorder, with every patient reported to have a positive outcome with the use of indomethacin [9,[20][21][22][23]. Additionally, one case report showed a successful treatment with low-dose aspirin resulting in remission in a 9-year-old who suffered from chronic PH [24]. The effectiveness of treating with indomethacin along with the typical shorter duration of each individual attack allows PH to be differentiated from cluster headaches [15•].…”

“…Acetylsalicylic acid has been shown to be efficacious during the early phases of CPH in some patients [46••] and possibly during the childhood phase of the disorder [65]. Partial success has also been reported with verapamil [46••, [106][107][108], steroids [108], and naproxen [46••].…”
