Successful Embolization Therapy through Reentry Tear in the Right Subclavian Artery for Treating Patent False Lumen in the Aortic Arch Formed after Type A Dissection Repair

Abstract: A 73-year-old woman had undergone hemiarch replacement with primary entry resection for treating acute type A dissection 6 years ago. Postoperative computed tomography (CT) showed a patent false lumen (FL) in the aortic arch and a reentry tear in the right subclavian artery. The remaining aortic arch enlarged, which resulted in formation of a 55-mm-diameter aneurysm. We performed reentry occlusion using embolization with glue and coil. The patient s clinical course after the procedure was uneventful, and subse… Show more

“…Furthermore, the subacute setting did not allow for the fabrication of a customized branched or fenestrated stent‐graft. However, we found a limited number of reports describing coil embolization as a feasible alternative treatment of arch aneurysms that developed after surgical repair of type A aortic dissection . To our knowledge, we present the first coiling procedure of an aortic arch pseudoaneurysm following stenting.…”

“…However, we found a limited number of reports describing coil embolization as a feasible alternative treatment of arch aneurysms that developed after surgical repair of type A aortic dissection. [10][11][12] To our knowledge, we present the first coiling procedure of an aortic arch pseudoaneurysm following stenting. Coil embolization is frequently used by interventional radiologists in the treatment of intracranial aneurysms.…”

“…In postoperative follow-up, preemptive endovascular interventions such as vascular plug, stent graft, coils, and glues might be a choice if the blood flow from the intimal tear to the false lumen is severe. 19 , 20 , 21 From this standpoint, the presence of a morphological parameter reflecting the internal pressure would help us to determine the most appropriate therapy strategy.…”
