Successful Transcatheter Closure of a Large Patent Ductus Venosus with the Amplatzer Vascular Plug II

Abstract: Patent ductus venosus is a rare form of congenital portosystemic shunt from the fetal umbilical vein to the inferior vena cava. The reported surgical treatments include ligation, banding, and liver transplantation. In addition, transcatheter closure with a coil, stent, or original Amplatzer vascular plug (AVP) has been reported. The AVP II, a redesigned version of the original vascular plug with a finer more densely woven nitinol wire and a large diameter (up to 22 mm) is available. This reported case is the f… Show more

“…Type IIa shunts are unique in that spontaneous closure has been reported in at least 13 young children [4,12,13]. When persistent, endovascular closure using coils, plugs, and/or stents has proven successful for many children with PDV [9,14,15] and other isolated type IIa shunts [16,17]. Liver mobilization typically allows for ligation of type IIa shunts without liver resection when coiling is not possible [18][19][20].…”

“…In the embolization of arteries, which are high-flow vessels, migration of the AVP is very rare (7). Internal iliac artery embolization is one of the most commonly reported applications of the AVP in arterial embolization, because if a common iliac artery aneurysm extends to the common iliac artery bifurcation in a patient with or without abdominal aortic aneurysm, simply extending the endograft into the external iliac artery beyond the internal iliac artery orifice could result in type 2 endoleakage (8).…”

“…In these cases, ADO is not suitable and the use of coils bears a high risk of complications such as residual leakage or embolization of the coil. In spite of the use of AVPs on a large scale [10][11][12] , some reports have claimed that the use of an AVP is contraindicated for the large tubular-type of PDA 13) . In this case, we tried using a larger size of AVP than is generally recommended and that brought about packing of the PDA.…”
