Successful treatment of a bronchial inflammatory pseudotumor by bronchoplasty in an 8-year-old boy: Report of a case

Abstract: We report herein the rare case of an 8-year-old boy in whom an inflammatory pseudotumor of the upper lobe bronchus of the right lung was successfully treated by bronchoplasty. A bronchoscopy was initially performed to investigate the cause of pulmonary atelectasis in the right upper lobe, which revealed a tumor in the right main bronchus. Thus, a thoracotomy followed by bronchotomy of the right main bronchus was carried out. The tumor was seen to have polypoid protrusion into the right main bronchus at the ori… Show more

“…Conservative surgery remains the treatment of choice, (1) although the locally aggressive behavior of the tumor occasionally requires the use of more radical procedures, such as pneumonectomy. Other reports in the literature (4,6) report favorable results when bronchoplasty is performed for the treatment of such tumors. In the present case, the proximity of the tumor to the middle lobe bronchus and its extension into the intermediate bronchus required that a bronchoplasty be performed in order to achieve tumor-free margins and preserve the middle…”

“…Similarly, for benign tracheal or bronchial tumors, we recommend lobectomy or segmentectomy with bronchial wedge resection rather than sleeve resection if the obstruction of the bronchus has resulted in irreversible destruction of the lung tissue and lobectomy or segmentectomy alone cannot resolve the problem. For example, Azevedo-Pereira AE ( 19 ), Galvez C ( 20 ), and Maeda M ( 21 ) used lobectomy or segmentectomy with wedge bronchoplasty for the treatment of bronchial glomus tumor, bronchial lipomas, and bronchial inflammatory pseudotumors, respectively.…”
