Successful treatment of Aerococcus viridans endocarditis in a patient allergic to penicillin

Abstract: Aerococcus viridans is a rare human pathogen that occasionally causes endocarditis. Most of the reported cases of endocarditis have been treated with penicillin. Here we describe a patient who was allergic to penicillin and was successfully treated with cefotaxime.

“…This organism is generally saprophytic and has rarely been encountered as a human pathogen [46]. There are case reports of urinary tract infection with bacteremia and endocarditis caused by Aerococcus viridans [7, 8]. Since Aerococcus viridans is usually recognized as susceptible to penicillin, the treatment protocol for aerococcal infection is generally started with penicillin or its derivates [9].…”

“…6,7 A. viridans causes meningitis, urinary system infection, osteomyelitis, septic arthritis, wound site infection, and more frequently bacteremia and infective endocarditis in immunocompromised patients. 8 Granulocytopenia, oral mucositis, history for long-term hospitalization, previous antibiotic use, invasive procedures, and implantations have been defined as primary risk factors for A. viridans related systemic infections. 2,6,7 Although source of contamination could not be clearly determined in the present case, we can say that it is a health-carerelated infection and comorbid immunosuppressive disease have contributed to this infection since he has been receiving hemodialysis for 6 years and had undergone invasive procedure short time ago.…”

“…Among the 30 strains of Aerococcus, Aerococcus viridans (83.3%) was mainly involved in the cellulitis of dental origin. The normal habitat of Aerococcus viridans is the natural environment [13]. However, in humans, it can be isolated from blood cultures (e.g.…”

“…A study in Tunisia noted involvement of Enterococcus in oral infections [14]. These bacteria are opportunistic pathogens, and they have been implicated in severe infections in immunocompromised as well as immunocompetent patients [13].…”
