Suprahyoid Pharyngotomy for excision of Laryngeal Venous Malformation

Abstract: We report an uncommon case of symptomatic venous malformation of the larynx in a 22-year-old man, and the successful surgical management thereof. A detailed radiologic investigation of the lesion was done, followed by surgical excision via a suprahyoid pharyngotomy using cold instruments. Long-term follow-up was maintained, and the patient had subjective and objective resolution of his symptoms and signs at 3 years postoperatively. We conclude that radiology can reveal hitherto unsuspected extensions of vascul… Show more

“…Cavernous hemangioma has been reported in the adult larynx. 12 So, the third case in the present series is quite uncommon. In case of tracheal involvement, hemoptysis can be a serious presentation.…”

“…Previously, sclerotherapy alone has been used when the size of the lesions has been below 2 cm 3. Surgical excision was used for a large lesion in a symptomatic 3-year-old with the addition of surgical tracheostomy for concurrent airway security 2. Our case adds to the evidence for even large LVMs in adult populations being treated with sclerotherapy.…”

“…Laryngeal vascular malformations (LVMs) are rare, and the literature is limited to small case series and reports. They are however usually symptomatic, and patients can present with exertional fatigue, hoarseness or bleeding 1 2. The LVMs reported in the literature vary in terms of size and aetiology.…”

“…1,3 However, there are numerous reports in the literature of the condition being diagnosed in children aged over 2 years and even in adulthood. [4][5][6] We therefore evaluated the symptoms of children diagnosed with laryngomalacia after the age of 2 years at Sheffield Children's Hospital.…”

“…Different endoscopic and open surgical techniques have been proposed, and these have included the use of KTP and Nd:YAG lasers and open excision of the venous malformation via the suprahyoid lateral pharyngotomy approach. [4][5][6] In this study, we report our experience with treating airway compromise due to vascular malformations of the larynx using a simple endoscopic technique involving electrofulguration.…”
