Objectives/Hypothesis: To analyze the clinical features, classification, and treatment of adult nasopharyngolaryngeal hemangioma (ANPLH).Study Design: Retrospective study.Methods: From February 2009 to May 2020, 101 patients with ANPLH were reviewed and analyzed.Results: Symptoms of ANPLH were frequently displayed as abnormal pharyngeal sensation and functional defection. According to lesion location, ANPLH was divided into five categories including nasopharyngeal, oropharyngeal, hypopharyngeal, laryngeal, and mixed types. The mixed type constitutes the highest portion, and the nasopharyngeal type is the least in our cohort. Most lesions could resect through natural cavity under endoscopy. Patients with mixed lesions had a higher rate of postoperative recurrence and planned multiple surgeries. Acceptable but not severe intraoperative and postoperative complications occurred in our patient cohort.Conclusions: Patients with ANPLH are always symptomatic and even functional defective, which can be classified into five categories based on lesion location. For these patients, endoscopic surgery through natural cavity is recommended to remove lesions with fewer complications and favorable clinical outcomes.

Intracranial aneurysm is a very common disease, which is an abnormal bulge on the wall of intracranial artery. It is the first cause of subarachnoid hemorrhage. With the development of imaging, the disease is easily diagnosed. A 60-year-old man was diagnosed with anterior communicating artery aneurysm and underwent aneurysm clipping. During routine induction of anesthesia, a nasopharyngolaryngeal hemangioma was visualized under a video laryngoscope. This poses a serious challenge for tracheal intubation. With the assistance of a video laryngoscope and local anesthesia with lidocaine, a 6.5# reinforced endotracheal tube was inserted. Nasopharyngolaryngeal hemangioma is a congenital vascular anomaly characterized by proliferation of endothelial cells. The incidence of infantile and childhood hemangiomas is low, and most spontaneously resolve by the age of 4 to 6 years. Adult nasopharyngolaryngeal hemangioma are extremely rare and are residual lesions from infancy and childhood. Although nasopharyngolaryngeal hemangioma does not clearly affect the quality of life of patients, intracranial hemangioma has caused blurred vision and decreased vision. Therefore, surgery for intracranial aneurysm was necessary for this patient. This case emphasizes the management of unplanned difficult airway of pharyngeal hemangioma and the emergency plan for hemangioma rupture and bleeding. Provide guidance and risk management procedures for nasopharyngotracheal intubation.

Management of Adult Hypopharyngeal Vascular Anomalies—A Descriptive Study

Objective: Given the rarity of adult hypopharyngeal vascular anomalies (AHVAs), no systematic studies are currently available. The aim of this study is to explore therapeutic strategies for AHVAs and determine the optimal timing for surgery in their treatment. Methods: We conducted a retrospective analysis of the data from patients diagnosed with AHVAs who underwent treatment using either bleomycin injection or excision methods in our department between September 2013 and November 2022. The study evaluated the curative efficacy, factors influencing efficacy, the size of AHVAs, and the selection of surgical methods. Results: A total of 23 adult patients underwent 30 distinct procedures. The study revealed that the choice of surgical methods significantly impacted therapeutic outcomes, with potassium-titanyl-phosphate laser, carbon dioxide (CO2) lasers, low-temperature plasma, and Holmium laser proving effective in treating AHVAs. The therapeutic effect was found to be worse for AHVAs involving multiple subzones compared to those with single subzone invasion. Notably, 3 patients underwent 2 surgeries: the first involved a bleomycin injection, followed by a complete surgical excision. Conclusions: The findings of our study emphasize the superior outcomes achieved through surgical excision when AHVAs are clearly delineated. Furthermore, we recommend opting for surgical excision after a reduction of AHVAs induced by bleomycin injection.
