Surgical strategies in patients with gallbladder cancer: Nihilism to optimism

Abstract: Surgery for gallbladder carcinoma, like other malignancies, has the potential to be curative only in local or regional disease. Pattern of loco-regional spread of disease dictates the surgical procedure. Radical surgery improves survival in early gallbladder carcinoma. The long-term benefit of aggressive surgery for advanced disease is unclear and may be offset by the high mortality and morbidity.

“…Therefore, the only chance for a cure lies in early detection and complete surgical resection. 5,20,21 In the current study, we investigated and analyzed the survival outcomes and clinicopathological characteristics of patients who underwent R0, R1, or Rx resection for T2 gallbladder cancer that invades the subserosal layer. The 5-year survival rate of T2 gallbladder cancer has been reported to be 49.8-78.3%.…”

“…[23] published an algorithm for the management of patients with IGBC, in which re-resection is not recommended for T1a carcinomas but is recommended for T1b and T2 considered together as one group. In the literature, it still is unclear whether patients with T1b carcinoma benefit from radical surgery, but there seems to be a general preference for a more aggressive approach.…”

“…The evidence database for the catalog was assembled only for CC arising in the intra-and extra-hepatic bile ducts. Our review did not include the management of gallbladder cancer, as several other comprehensive articles had already covered this topic [7][8][9][10] . To identify all potential papers, we searched medical subject headings reported in Table 1.…”
