Surgical Treatment and Prognosis of Esophageal Cancer After Distal Gastrectomy

Abstract: The patients with partial gastrectomy for more than 5 years, having upper gastrointestinal symptoms, should be considered having the risk of esophageal cancer associated with gastric remnant. For patients with a history of distal gastrectomy, a vascularized pedicle colonic conduit was most commonly used for esophageal substitution. Surgical efficacy was similar with the no-gastrectomy group in early stages I-II of esophageal cancer associated with gastric remnant but was lower compared with the no-gastrectomy … Show more

“…Duodenal reflux is also known to be carcinogenic (48), but requires both gastroesophageal and pyloric sphincters to be defective. Distally located ESCC has been associated with severe hypochlorhydria (41) and gastrectomy (49, 50), the latter of which vastly increases the propensity for duodenal reflux (51). Countering this are our exploratory analyses which provided similar estimates of association between gastric atrophy and ESCC across esophageal sites (upper, middle, and lower thirds; Supplementary Table 1).…”

“…The literature details various replacements. Colonic transplantation is the preferred substitution by the majority of surgeons, owing to its reliability and acceptable survival rate . Jejunal interposition reconstruction is used less frequently in cases with concomitant colonic diseases .…”
