Synovial recesses of the knee: MR imaging review of anatomical and pathological features

Abstract: The knee joint is a complex anatomical structure playing host to a wide variety of pathological processes. Knowledge of the anatomy of the synovial recesses and plicae relating to the knee is important, as the appearance of pathology in these unusual locations may, even for commonly encountered conditions, lead to diagnostic uncertainty. This review article discusses the magnetic resonance imaging (MRI) anatomy of the knee joint with an emphasis on the synovial recesses and plicae. The MRI appearance of a vari… Show more

A variety of disorders such as overuse [10], trauma [11], infection, hemorrhage, arthropathy, internal joint derangement, villonodular synovitis [12][13][14], and synovial osteochondromatosis [15] may cause the accumulation of fluid within the bursae and thickening of the synovia, allowing visualization on sonography.…”

“…The knee bursae are numerous and, in the absence of pathological changes, rarely apparent on MR imaging [ 3 ]. The knowledge of the locations of synovial bursae is important since in pathological conditions, they may become apparent and a cause for misdiagnosis or diagnostic uncertainty [ 2 ]. The classifi cation of the bursae is made on their anatomic location (Table 8.1 ).…”

The anterior and posterior cruciate ligaments, menisci, and the infrapatellar fat pad are located "extrasynovially" which means that they are located outside the abovementioned synovial compartments.…”

“…The suprapatellar recess or bursa is a pouch formed by the articular capsule of the knee possessing its own synovial lining (8), while the medial parapatellar and lateral parapatellar recesses are extensions thereof in the medial parapatellar and lateral parapatellar areas, respectively. The parapatellar recesses are recommended among the standard scans of the knee in the guidelines on the use of sonography in rheumatology (6) and have been utilized for individual measurement of fluid volume in the knee by several investigations utilizing ultrasound, as well as other imaging modalities (9 -12).…”
