Systematic review and meta-analysis of second-generation antidepressants for the treatment of older adults with depression: questionable benefit and considerations for frailty

Abstract: BackgroundFrail older adults are commonly prescribed antidepressants. Yet, there is little evidence to determine the efficacy and safety of antidepressants to treat depression with concomitant frailty. To better understand this issue, we examined the efficacy and safety of second-generation antidepressants for the treatment of older adults with depression and then considered implications for frailty.MethodsDue to the absence of therapeutic studies of frail older adults with depression, we conducted a systemati… Show more

“…In the field of treating depression, one of the most recent metaanalyses showed a limited efficacy of pharmacological treatment of depression in older patients, for whom there is virtually no statistically significant difference between the treatment with an antidepressant and a placebo. Additionally, the effectiveness of treating depression in the elderly population is twice as low as in people in early and middle adulthood, and is 45.3% and 40.5% in the group of people treated with an antidepressant or a placebo, respectively [2].…”

“…Other antidepressants, such as tricyclic antidepressants (TCAs), serotonin and norepinephrine reuptake inhibitors (SNRIs) and mirtazapine are associated with a similar or higher risk of a number of adverse events, including sedation, confusion, urinary retention, cardiovascular and gastrointestinal issues [16], and are generally avoided in these patients. Moreover, the magnitude of beneficial effects of antidepressants in late-life depression has been questioned [17], making it hard for clinicians to accurately balance benefits and risks.…”
